Objective and Significance:By studying the expression levels of S100βprotein and lipoprotein-related phospholipase A2(Lp-PLA2)in patients with acute cerebral infarction(ACI),The relationship between S100βprotein and Lp-PLA2 and the severity of ACI is further explored,so that the research results can provide reliable results for ACI as soon as possible,early screening,early diagnosis,early assessment and early treatment.Based on the clinical reference,it provides a well-founded guidance for clinical treatment of ACI,which aims to reduce the mortality and disability rate of ACI patients,and further improve their quality of life and quality of life.Research Methods:From January 2018 to January 2019,patients in the Department of Neurology of Shaanxi People’s Hospital were collected as the study subjects,and clinical data related to the experiment were collected.According to the criteria of"Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke in China 2014",the study subjects were divided into two groups:(1)Cerebral Infarction Group(ACI Group):Computed Tomography There were 114 cases of acute infarction(n=114)in CT or MRI,including 59 males and 55 females,with an average age of(56.05±8.82)years.According to the Pullicino calculate the volume of infarcts formula and group them(V<5cm~3was a small infarction group,and 5≤V<10cm~3 was a middle infarction group,V≥10cm~3 was the large infarction group).At the same time,according to the NIHSS score,the neurological function of ACI patients was assessed;(2)Non-cerebral infarction group(NACI group):There were 103 cases without abnormal imaging results on cranial MRI,including 60 males and 43 females,with an average age of(55.75±9.86)years.In accordance with the relevant provisions of medical ethics,the levels of serum S100beta protein and Lp-PLA2 were measured by ELISA.At the same time,the general data and biochemical indicators of all the observers were collected,including sex,age,drinking history,smoking history and past history.Finally,SPSS 19.0 software was used to analyze and compare the general data and biochemical indicators of the two groups.The results were all represented by P<0.05.Results:1.There were no significant differences in age,sex,drinking history,smoking history and diabetes mellitus between ACI group and NACI group(P>0.05),but there were significant differences in hypertension,total cholesterol and low density lipoprotein(P<0.05).2.Compared with the concentration of S100beta and Lp-PLA2 in NACI group,the concentration of serum S100beta protein and Lp-PLA2 in ACI group increased significantly(t=16.512,P<0.05;t=13.995,P<0.05).3.Bivariate logistic regression analysis was conducted with hypertension,Lp-PLA2,LDL-C and TC as independent variables.The results showed that Lp-PLA2,LDL-C and TC were independent risk factors for ACI(OR=2.735,OR=1.600,OR=1.269,P<0.05).4.The levels of serum S100beta protein and Lp-PLA2 were(0.176±0.043)ng/ml and(170.18±49.12)ng/ml in the small infarction group,0.184±0.034)ng/ml and(195.29±45.64)ng/ml in the middle infarction group and(0.198±0.030)ng/ml and(220.96±63)ng/ml in the large infarction group,respectively.There was statistical significance(F=3.749,P<0.05;F=9.135,P<0.05).5.Pearson correlation analysis was used to analyze the correlation between serum levels of S100 beta protein and Lp-PLA2 and NIHSS score.The results showed that there was a positive correlation between them,and the difference was statistically significant(r=0.552,P<0.05;r=0.726,P<0.05).6.ROC curve analysis showed that the area under the curve of S100 beta protein was71.6%(95%CI:0.596-0.836,P<0.01).The sensitivity and specificity of diagnosis of ACI were 66.2%and 71.9%respectively.The area under the curve of Lp-PLA2 was75.3%(95%CI:0.655-0.852,P<0.01),and the sensitivity and specificity of diagnosis of ACI were 86.8%and 57.9%.Conclusions:1.S100beta is related to the occurrence of ACI.2.Lp-PLA2 is associated with the occurrence of ACI and is a risk factor for ACI.3.Serum S100beta level was positively correlated with ACI infarct volume and corresponding neurological deficits.4.Serum Lp-PLA2 level was positively correlated with ACI infarct volume and corresponding neurological deficits.5.S100beta and Lp-PLA2 have good predictive value for ACI.
