Objective: By observing Sinus CTs,this study analyzes the correlation between Nasal Septum Deviation and Chronic Rhinosinusitis Without Nasal Polyps(CRSsNP)and Inferior Turbinate Hypertrophy.It provides effective preoperative evaluations of clinical sinus surgery to avoid postoperative complications.Methods: 238 CT images were collected from patients with nasal septum deviation,then images were observed and measured using software-based Multi-Planar Reconstruction(MPR).The deviation angles of the patients were recorded and then classified into three categories based on deviation angle: light,moderate and severe,i.e.,light for angles ≤8°,moderate for angles between 8 and 15°,and severe for angles≥15°.Patients were divided into CRSsNP group and non-CRSsNP group to study and analyze the relationship between nasal septum deviation and CRSsNP.The wide side nasal cavity opposite to the deviation direction of nasal septum in each group was set as the experimental group,and the narrow side nasal cavity on the same side as the deviation direction of nasal septum was set as the control group.Lund-Machay CT scoring system was used to evaluate the sinusitis in CRSsNP group.Compared the constituent ratio and severity of sinusitis of both experimental and control groups,and analyzed correlation between the nasal septum deviation angle and the severity of CRSsNP.The medial mucosa thickness,lateral mucosa and inferior turbinate bone of inferior turbinate in non-CRSsNP group were also measured.This study also compared the thickness of inferior turbinate in both experimental and control groups to analyze the correlation between the of nasal septum deviation angle and inferior turbinate hypertrophy.Results: Among 238 cases of patients with nasal septum deviation,125 cases(52.52%)has CRSsNP and 113 cases(47.48%)has non-CRSsNP.The degree of nasal septum deviation had statistical significance between the CRSsNP group and the non-CRSsNP group(Ρ=0.015).The constituent ratio of CRSsNP was higher in patients with moderate and severe nasal septum deviation.There was no significant difference in the direction of nasal septum deviation between CRSsNP group and non-CRSsNP group(Ρ= 0.197).In the CRSsNP group,there were 361(57.76%)turbid sinuses in the wide nasal cavity of the experimental group,and 310(49.60%)in the narrow nasal sinus in the control group.The difference between the two groups was statistically significant(Ρ<0.05).There was no correlation between the angle of nasal septum deviation and the Lund-Mackay CT score of sinus in patients with CRSsNP(Ρ=0.628).In the non-CRSsNP group,the medial mucosa,lateral mucosa and bone thickness of the inferior turbinate were different between the experimental group and the control group(Ρ<0.001),and the medial mucosa was thickened.There was a statistically significant correlation between the angle of the nasal septum deviation and the thickness of the inferior turbinate bone in the experimental group(r=0.370,Ρ<0.001.There was no correlation between the angle of the nasal septum and the thickness of the medial and lateral mucosa of the inferior turbinate of the experimental group.((49)>0.05).Conclusion: 1.The constituent ratio of CRSsNP is higher in patients with moderate and severe nasal septum deviation,the constituent ratio and severity of CRSsNP in the wide side of nasal septum deviation are higher than those in the narrow side,there is no correlation between the deviation angle of nasal septum and the severity of CRSsNP.2.The inferior turbinate mucosa and inferior turbinate bone on the wide side of nasal septum are thicker than those on the narrow side,mainly medial mucosa;The deviation angle of nasal septum is positively correlated with the thickness of inferior turbinate bone on the wide side of nasal cavity,but not with the thickness of medial and lateral mucosa of inferior turbinate. |
