Objective:To systematically evaluate the clinical efficacy and safty of low-dose roxithromycin in patients with stable chronic obstructive pulmonary disease.Methods : The databases of PubMed,Medline,Cochrane library,CNKI,VIP Database and WanFang Database were retrieved with computer for searching the randomized controlled trials of low dose roxithromycin in patients with stable chronic obstructive pulmonary disease from database establishment time to Nov,2017.and meanwhile the references of included studies were retrieved manually.The literature was screened,data was extracted and quality was reviewed independently by two reviewers according to inclusion and exclusion criteria,the results of studies were analyzed using RevMan 5.3 software.Results:A total of 925 participants were enrolled in the 11 RCTs,with 462 patients in the trial group and 463 in the control group.The Meta analysis showed that pulmonary function [MD=0.81,95% CI:0.45~1.17,P<0.0001],the quality of life and sixminute walk distance[MD=37.08,95% CI:13.83~60.33,P=0.002]of trial group were significantly better than those of control group,there was statistical significance.Difficulty breathing score [MD=-1.22,95% CI:-1.89~-0.55,P=0.00004],amount of acute exacerbation[OR=0.23,95% CI : 0.15~0.36,P < 0.00001 ],inflammatory cytokines and cells of trial group were significantly lower than those of control group,there was statistical significance.There was no significant difference in adverse reactions and sputum volume in two groups.Conclusions : Low-dose roxithromycin have good therapeutic efficacy for stable COPD and effectively improve the quality of patients’ life,there was no serious adverse reactions,So we recommend focus on the clinical application of roxithromycin.
