| Purpose:Damp turbidity is one of the common causes of TCM.Its pathogenesis is extremely extensive,clinical manifestations are complex,involving many viscera,the course of disease is lingering,and easy to repeat,bringing great pain to patients.However,the differentiation and treatment of dampness and turbidity is a difficult and doubtful point in clinic at present.The main reason lies in the fact that many physicians who have studied dampness and turbidity in the past dynasties have either partially discussed the dampness and turbidity in a certain viscera or only described the etiology,pathogenesis and part of the disease location,thus lacking systematic understanding of dampness and turbidity.Therefore,the purpose of this study is to preliminarily define the connotation and extension of dampness through literature and clinical research,elaborate the pathogenesis and pathogenic characteristics of dampness,sort out the main syndrome types,treatment methods and prescriptions,so as to improve the clinical efficacy.Method:1.Using the method of literature collation and research,taking the works of TCM such as Warm Lun,Differentiation of epidemic febrile diseases,Warm Jingwei as the research object,first of all,combing out the chapters about dampness in the above literature,and secondly,taking the diseases caused by dampness and turbidity as the outline and the causes of the corresponding articles as well as the paragraphs and sentences about "dampness and turbidity","turbidity","turbidity and phlegm turbidity".(or combined with etiology),pathogenesis,disease to be analyzed,and clear labeling in the form of card removal,sorting out the main clinical syndromes of dampness and turbidity diseases,clinical manifestations of each type of syndromes and the corresponding prescription medication.On this basis,try to define dampness and turbidity,and preliminarily elaborate the paths and pathogenic characteristics of dampness and turbidity.2.Using the method of clinical research,255 cases of damp turbidity were selected according to inclusion criteria and exclusion criteria in the medical records of associate professor of Qihuang outpatient department of traditional Chinese medicine from 2011 to 2018.The causes,pathogenesis,causes and clinical symptoms of damp turbidity were analyzed and recorded.The main types of symptoms of damp turbidity were summarized,including prescriptions and medicines.The common diseases of dampness and turbidity were summarized.Results:1.Literature research:In ancient Chinese medicine works,there are a lot of discussions about Wet Turbidity pathogenic characteristics,pathogenesis and specific articles on syndrome differentiation and treatment.The results are as follows:(1)Definition of turbidityWet turbidity refers to a kind of evil gas which is produced by the organism’s feeling of the turbidity of nature or the abnormal metabolism of water and moisture in the body.It is characterized by the heavy,dirty and easily adhered to the local part of the human body.(2)PathogenesisExternal dampness invades the human body by the basic mode of "starting from the upper coke,ending from the lower coke" and "from theoutside to the inside".Endogenous dampness is easily transformed into phlegm turbidity,stasis turbidity and toxicity in the body,and then spreads over all organs of the body.It is omnipresent with the movement of Qi and blood and involves a wide range of pathological changes.(3)Pathogenic characteristics of dampness:(1)Damp,turbid and dirty,easy to appear secretions or excreta unclean;(2)wet and sticky,easy to clamp phlegm and blood stasis;(3)damp and turbid,clamp yin,easily obstruct the heat of Qi mechanization,become turbid and poisonous over time;(4)humidity has certain fluidity,and its clinical manifestations are complex and changeable.(4)The main types of syndromes(in the name of Tang)are:(1)Banxia Xiexin Decoction Remove Ginseng,Ginger,Date and Licorice,plus Bitter orange almond soup syndrome;(2)Baitong Decoction Certificate of Zanthoxylum bungeanum and Aconitum Fructus;(3)Shujiao Jiuzhong Decoction Syndrome;(4)Sanxiang Decoction Syndrome;(5)Angong Niuhuang Pill plus Fuling Peel Decoction;(6)Laifutan syndrome;(7)Xuanqing Daozhuo Decoction;(8)Five additions and subtractions of Zhengqi Dispersion Syndrome;(9)Glycyrrhizae Decoction for Purging Stomach-Fire plus Sophora flavescens and Realgar Decoction syndrome;(10)Shrinking Spleen Decoction Syndrome;(11)Decoction of Capejasmine and Fermented Soybean with Platycodon grandiflorum,Fructus aurantii syndrome;(12)Pu ash syndrome;(13)addition and subtraction Sanjia San syndrome.2.Clinical research:(1)dampness and turbidity are often accompanied by wind,phlegm,heat and blood stasis,among which wind is 91.8%,heat is 89.4% and blood stasis is 45.4%.Depression,heat stasis and deficiency due to reality are the most common pathogenesis,of which depression is 56.07%,blood stasis is36.86%,deficiency due to reality is 10.58%.Less yang,Yang-Ming and Jueyin are the most common pathogenesis(74.11%,60.78%,43.92%respectively).(2)There are 9 types of clinical syndromes of common damp turbidity,which are:(1)dampness,turbidity,stagnation and heat,yang-ming-qi syndrome;(2)Turbidity of phlegm and stagnation of heat,the original syndrome of Shaoyang membrane;(3)Dampness,turbidity,stagnation and heat,and syndrome differentiation of Shaoyang qi;(4)Dampness,turbidity,stasis and heat,syndrome of Jueyin and blood;(5)Cold turbidity,depression and heat,syndrome differentiation of Taiyin Qi;(6)Rheumatism,turbidity and heat,Shaoyang Yangming Qi syndrome;(7)Shaoyang Yangming,Qi and Blood Syndrome,Rheumatism,Turbidity and Stasis Removing Heat Syndrome;(8)Shaoyang Jueyin is characterized by rheumatism,turbidity and stasis,dampness and heat damaging yin;(9)Yangming Jueyin Rheumatism,Turbidity and Heat,Liver Depression and Blood Deficiency Syndrome.Conclusion:1.Wet turbidity refers to a kind of evil gas which is produced by the organism’s feeling of the filthy substances of nature or the abnormal metabolism of water and moisture in the body.It is characterized by heavy,filthy and easily adhered to the local part of the human body.2.Through literature research and clinical research,the etiology of dampness and turbidity is not a minority of clinical diseases,and it has independent clinical manifestations,diagnostic significance and classification of syndrome and treatment in the process of disease diagnosis.3.The classification of syndromes and therapies in literature research and clinical research is different.In ancient literature,the types of syndromes and therapies of cold,damp,turbid and stagnant heat are more common,while in clinical research,the types of syndromes and therapies of damp-heat,turbid phlegm,stasis and stagnation are more common.It is necessary to conduct in-depth clinical and experimental studies on theetiology of dampness and turbidity in order to improve the clinical efficacy of dampness and turbidity diseases. |
