| Objective: By collecting the clinical and pathological data of thyroid papillary carcinoma complicated with Hashimoto’s thyroiditis,this paper analyzes and discusses the related factors of cervical lymph node metastasis in patients with thyroid papillary carcinoma complicated with Hashimoto’s thyroiditis,and evaluates the risk of lymph node metastasis in various aspects,so as to provide certain theoretical basis for the clinical treatment of such patients.Methods: Collected in the affiliated hospital of Qingdao university in 2014-2018 final pathologic diagnosis of papillary thyroid cancer complicated with Hashimoto’s thyroiditis patients,a total of 308 cases,it be divided into the neck lymph node metastasis group(132 cases,42.9%)and the non-neck lymph node metastasis group(176 cases,57.1%),to analyze the clinical data,including the patient’s gender,age,tumor size,single multifocal,whether outside invasion,preoperative TSH level,whether to have thyroid cancer family history,and then discusses the above factors a merger with thyroid papillary carcinoma hashimoto thyroiditis patients with cervical lymph node metastasis.Results: Single factor analysis showed that multiple lesions(P < 0.01),age(P = 0.036),tumor size(P < 0.01),statistically significant(P < 0.05),with lymph node metastasis compared between the groups with and without lymph node metastasis onset age is younger,tumor diameter larger,multiple lesions is more,and the remaining clinical data were not statistically significant(P > 0.05);Multivariate Logistic regression analysis showed that multiple lesions(P < 0.05,OR=4.348,95%CI : 2.357-8.019),tumor size(P < 0.05,OR=4.927,95%CI:2.357-8.019)and age(P < 0.05,OR=0.971,95%CI:0.948-0.995)were related factors for cervical lymph node metastasis in patients with thyroid papillary carcinoma complicated with Hashimoto’s thyroiditis.Conclusion: Patients with thyroid papillary carcinoma complicated with Hashimoto’s thyroiditis with younger age,larger tumor diameter and multiple lesions have a higher risk of cervical lymph node metastasis than those with older age,smaller tumor diameter and single lesion.Therefore,the cervical lymph nodes of patients should be fully evaluated to determine the scope of lymph node dissection. |
