Objectives To explore the effect of intervention based on behavioral change theory on pressure ulcer cognition and care behavior of main caregivers of high-risk patients with pressure ulcer,so as to provide scientific basis for better prevention of pressure ulcer and improvement of care behavior of main caregivers.Methods The main caregivers of high-risk patients with pressure ulcers hospitalized in the Department of Neurosurgery,Affiliated Hospital of North China University of Technology from October 2017 to September 2018 were selected as the subjects of study.The main caregivers who met the inclusion criteria were numbered according to the order of admission of the cared patients.The number of the main caregivers was odd in the control group and even in the intervention group.The control group received routine health education;the intervention group received intervention based on behavioral change theory,including pre-intention stage,intention stage,preparation stage,action stage and maintenance stage.The patients were intervened for one month from admission.Before and after the intervention,two groups of subjects were assessed with pressure ulcer cognitive level scale,pressure ulcer care behavior scale,care burden scale and general selfefficacy scale.Results 1 In this study,150 primary caregivers who met the inclusion and exclusion criteria for high-risk patients with pressure ulcer were selected as the study subjects,and the subjects were divided into control group and intervention group with 75 cases each.During the intervention,4 cases in the control group were lost,2 cases in the intervention group were lost,and 1 case in the care group died.The final control group included 71 cases and intervention group 72 cases.2 Before intervention,there was no significant difference between the two groups in the basic situation of the main caregivers and the cognitive and behavioral status of pressure ulcer(pressure ulcer awareness,care behavior,care burden,self-efficacy),indicating that the balance of the two groups of main caregivers before intervention was comparable.After intervention: 3 Comparisons of pressure ulcer cognition among the three main caregivers in the intervention group were higher than those in the control group(P < 0.05),and the cognitive level of pressure ulcer in the intervention group was significantly higher than that in the control group(P < 0.05).4 Comparing the caring behavior of the main caregivers of the two groups: the total score and the three dimensions of turning over behavior,skin care and nutritional support of the intervention group were higher than those of the control group,the difference was statistically significant(P < 0.05),and there was no significant difference in the dimension of wound care(P > 0.05).5 The scores of the intervention group were lower than those of the control group(P < 0.05),and the scores of the intervention group were significantly lower than those of the control group(P < 0.05).6 The self-efficacy of the main caregivers in the intervention group was higher than that in the control group(2.27+0.31)(P < 0.05).Conclusions 1.Intervention based on behavioral change theory can effectively improve the cognitive level of the main caregivers of high-risk patients with pressure ulcer and improve their pressure ulcer care behavior.2.Intervention based on behavioral change theory can reduce the burden of care of the main caregivers of high-risk patients with pressure ulcer and improve their self-efficacy.Figure0;Table12;Reference 94...
