| Objective:To investigate the clinical efficacy of endovascular-based treatment in iliac vein compression syndrome complicated with venous ulcer of lower limbs.Methods:The clinicail data and follow-up data of 36 patients with iliac vein compression syndrome and venous ulcer treated in our hospital from March 2015 to March 2018 were etrospectively analyzed.The venous ulcer of all patients was active.Double lower limbs color Doppler ultrasound and double lower limbs CTV and other examinations were performed before the operation and evaluated the degree of iliac vein stenosis.All of them were consistent with the diagnosis of iliac vein compression syndrome.Endovascular treatment were performed on all patients,and superficial vein surgery were performed on patients with obvious varicose veins,and medication,pressure and wound care were given after the opreation.Follow-up visit of patients were given to check parameters like healing status of ulcer,preoperation and postoperation venous clinical severity,life quality and possible complications and so on.Results:All patients successfully underwent Percutaneous transluminal angioplasty sttenting,48 venous stents were implanted successfully.21 cases were only implanted in left iliac vein,4 cases of only right ones and 11 cases of both sides(1 case of three stents).The success rate was 100%,15 patients with obvious varicose veins underwent superficial venous surgery.The healing rate of active ulcers at 1,3,6 and 12 months after the operation was respectively 80.56%、91.67%、97.22%、100%.The healing status of ulcer,venous clinical severity and life quality scores after operation were significantly improved than the preoperative,and the differences were statistically significant(P<0.05).The venous patency rate was 100% in 1,3 and 6 months after the operation.The left iliac and femoral venous thrombosis was found in case at 10 month after the operation,which was removed after interventional therapy.The perioperative complication rate was 5.56% and the long-term complication rate was 2.78%.Conclusion:Endovascular-based treatment for iliac vein compression syndrome combined with venous ulcer are safe and effective.It can effectively accelerate the healing of ulcer,improve the clinical symptoms and the life quality. |
