How To Improve The Quality Of Emergency Treatment

Objective The purpose of this study is to improve the quality of emergency treatment from two aspects: "EDLOS" and "pre hospital LSFA" :To investigate the effect of ED visits,the disease constitution of ED,the pressure of emergency treatment and the EDLOS in a tertiary teaching hospital,along with the rapid growth of the elderly population,and the analysis of risk factors would affect the prognosis of patients.Investigate knowledge of LSFA and attitude to performing bystander LSFA among the youth in shanghai to provide some new ideas for improving the public LSFA knowledge and “bystander” emergency ability by analysis of associated factors of LSFA.Methods 1)Data of patients presented to an independent-design ED data platform in a tertiary hospital from Jan.2014 to Dec.2016,including demographic information(gender,age,time,etc.),clinical characteristics(diagnosis,departments,etc.),EDLOS≥6h and outcome.Mann-Whiteny U,Kruskal-Wallis H test and the Cox proportional hazards model were adopted to identify the non-normal data.2)Self-designed questionnaires that were named “Life supporting first aid KAP for teenagers in Shanghai” were distributed to the youth in Shanghai,China.About more than 1000 teenagers were selected,and analysis of T-test variance,rank-sum test and chi square test were used for statistical analysis.P<0.05 was statistically significant.Results 1)The elder patients(≥65y)was 8980(54.96%).The elder proportion in ED was much larger than that was in Shanghai.The elder patients’ EDLOS was significantly longer(P<0.05).The chronic aging diseases and degenerative diseases were also rapidly increased in elderly patients.2)The first-aid knowledge score of the youth in Shanghai was poor.Only 51% of high school students in Shanghai have received professional first-aid training courses.On the contrary,the attitudes towards students to learning the first aid were positive(85.5%)and willing(75.6%).Conclusion 1)The increasing proportion of elderly patients in ED is the main reason leading to the pressure of emergency treatment.The elderly patients who had chronic diseases prolong the EDLOS and cost the most medical resources of emergency treatment.2)The main reason of Shanghai high school students’ poor LASF score was that first aid education was seldom provide by schools、medical institutions,and whole society.Thus,it is urgent for us to spread first-aid knowledge among high school students,thereby improving execution rate of bystander first-aid so as to improve pre-hospital survival rate.And this is an effective way to improve emergency treatment quality.
