Construction Of Nursing Sensitive Quality Indicators System In Emergency Department

Objective The purpose of this study is to build a scientific,reasonable and applicable index system of emergency department nursing quality sensitivity based on evidencebased,"structure-process-result" three-dimensional quality model as theoretical basis,using Delphi method and analytic hierarchy process,so as to provide theoretical basis and practical guidance for scientific management of emergency department nursing quality.Methods Using evidence-based nursing method of Johns Hopkins to synthesize evidence,at the same time,using focus group interview method to screen indicators,and initially drawing up the index letter inquiry volume;Delphi method was used for two rounds of expert correspondence to revise and determine the sensitivity index system of nursing quality in emergency department.Finally,the weight calculation and consistency test of the sensitivity index system of nursing quality in emergency department were carried out by AHP.Results1.Through two rounds of Delphi method,a sensitivity index system of nursing quality in emergency department was constructed,including 3 first-class indexes,10 second-class indexes and 27 third-class indexes.After two rounds of inquiry,the recovery rates of expert questionnaires were 100% and 92.86%,respectively,which indicated that the inquiry experts who participated in this study were highly motivated.The expert authority coefficients are 0.85 and 0.88,respectively,which indicates that the selected inquiry experts have certain authority;The overall Kendall harmony coefficients of the two rounds of indicators are 0.386 and 0.372（P < 0.001）,which shows that the opinions of the inquiry experts on the indicators are relatively concentrated,and the inquiry results are relatively scientific,reasonable and credible.2.Using AHP analytic hierarchy process,on the basis of index importance score,the weight calculation and consistency test of indexes at all levels are carried out.Results All levels of indicators met the consistency requirement（CR< 0.1）,among which the first level indicators ranked as process indicators,structure indicators and result indicators,and their weights were 0.4934,0.3108 and 0.1958 respectively.Among the secondary indicators,the priority of structural index combination weight is the disease grade and bed usage of emergency department patients;First aid drugs,instruments and equipment management,and nursing evaluation rank higher in the process combination weight;Results The priority of index combination weight was nursing adverse events,nurse outcomes and satisfaction.Conclusion According to the "structure-process-result" three-dimensional quality model,this study combined Delphi method and analytic hierarchy process to construct the sensitivity index system of nursing quality in emergency department,which ensured the rationality and scientificity of the results of experts’ letters and inquiries,and provided a theoretical basis for the evaluation and scientific management of nursing quality in emergency department.
