| Purpose: Based on the theory of traditional Chinese medicine and modern medicine,the etiology,pathogenesis and treatment of recurrent oral ulcer were discussed.The clinical efficacy and safety of Modified Xiehuang Decoction were evaluated by observing the improvement of symptoms of recurrent oral ulcer(pathogenic fire hidden in spleen and stomach).Method: From December 2017 to December 2018,68 patients with recurrent oral ulcer(pathogenic fire hidden in spleen and stomach)were selected from the digestive clinic of the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine.According to the random number table,they were divided into two groups.After eliminating and shedding cases,61 cases were observed,in which 30 cases in the treatment group while 31 cases in the control group.The treatment group was given Modified Xiehuang Decoction orally,while the control group was given Vitamin C and Vitamin B2 for 2 weeks.On the fifth day of treatment,the changes of TCM syndromes 、pain index and the overall curative effect of the two groups were evaluated.After the treatment,the healing time of ulcer was recorded.The effective cases were treated and the relapse situation was followed up by telephone after 3 months.The data were analyzed by SPSS 20.0,and the efficacy and safety of the treatment were compared.Result:1.Comparison of TCM syndrome integrals: the effective rate of treatment group was 90.0%,while that of control group was 70%.The effective rate of treatment group was better than that of control group(P < 0.05).2.Improvement of main symptoms: Compared with before treatment,the scores of main symptoms in both groups were decreased(P < 0.05).The treatment group was better than the control group in improving ulcer area(P < 0.05).There was no significant difference in improving ulcer number,ulcer congestion and local burning sensation between the two groups(P > 0.05).3.Improvement of secondary symptoms: Compared with before treatment,the scores of secondary symptoms in the treatment group were lower(P < 0.01);in the control group,the scores of thirst,appetite for drinking,stool,tongue and vein were lower(P < 0.01),while there was no significant change in hot breath and bad appetite(P > 0.05);in the treatment group,the scores of thirst,appetite for drinking,stool and tongue and vein were better than those in the control group(P < 0.05).4.Comparison of local efficacy: the treatment rate of local lesions in the treatment group was 93.3%,and that in the control group was 70.9%.The treatment rate of local lesions in the treatment group was better than that in the control group(P < 0.01),with statistical significance.5.Ulcer healing time: Compared with before treatment,the healing time of ulcer in the two groups was shortened(P < 0.05),but the healing time in the treatment group was significantly shorter than that in the control group(P < 0.01).6.Comparison of ulcer pain index: Compared with before treatment,the degree of ulcer pain in both groups decreased(P < 0.01),but there was no significant difference between the two groups(P > 0.05).7.Relapse rate comparison: the treatment group recurrence rate 11.1%,the control group recurrence rate 37.0%,the treatment group in reducing the recurrence rate is better than the control group(P < 0.05).8.Safety comparison: There was no obvious abnormality in physical examination and laboratory test before and after treatment in the two groups,and no adverse reactions occurred during the whole treatment process,indicating that Modified Xiehuang Decoction 、vitamin B2 and vitamin C had good safety.Conclusion:Modified Xiehuang Decoction is effective in treating recurrent oral ulcer(pathogenic fire hidden in spleen and stomach).It can effectively improve the symptoms of oral ulcer and shorten the course of the disease,reduce the recurrence rate in the near future,and has good safety.It is worth promoting. |
