Objective:To evaluate the effects of epidural labor analgesia on neonatal oxygenation.Methods:60 cases of primipara,aged 22 to 33 years old,ASA grade I or II,full-term and single-head pregnancy were enrolled,the primiparas who met the standard voluntarily epidural labor analgesia were assigned to group E,the primiparas who voluntarily gave up labor analgesia were divided into group S,(n=30 each group).Group E:When the cervical orifice was dilated to 3cm,the L2-3-3 intervertebral space was subjected to epidural puncture.After successful puncture,the tube was placed on the head side,and 3 ml of1%lidocaine was injected(experimental dose)and several minutes were observed to excluding the catheter from the vascular or subarachnoid space,and then 8-12ml of the mixture of 0.1%ropivacaine with 0.25ug/ml sufentanil was injected as the first dose to obtain a satisfactory effect followed by PCA pump,fetal placenta delivered,suture wounds completed followed by the withdrawal of the tube;group S:the conventional natural delivery procedures were performed.The VAS scores of the two groups and the Apgar scores at 1 and 5 min after birth were recorded.The umbilical cord was ligated immediately after the delivery of the fetus and 3 ml of the umbilical vein blood was taken from the umbilical cord for blood gas analysis.The umbilical arterial blood was taken and placed in biochemistry tubes,samples were centrifuged and plasma was taken and stored in an ultra-low temperature freezer at-80°C.The concentration of IMA and albumin in the serum was measured by Elisa method.Analysis was performed using SPSS 13.0 statistical software.Results:The VAS scores of the E group was significantly lower than those of the S group When the cervical orifice opened to 6,8,and 10 cm(P<0.05).There were no significant differences in Apgar score,umbilical vein blood gas analysis and umbilical arterial blood IMA concentration,albumin concentration,and IMA/albumin ratio between the two groups.Conclusion:Epidural labor analgesia clearly relieved the pain during maternal delivery,and had no significant effect on Apgar score,umbilical vein blood gas analysis and umbilical arterial blood IMA concentration,albumin concentration,and IMA/albumin ratio.Epidural labor analgesia has no effect on neonatal oxygenation,and the analgesic effect is exact,it is a safe and effective method of labor analgesia.
