Objective:Endometrial polyp(endometrial polyps,EP)refers to the local overgrowth of the endometrium,protruding from the intimal epithelial vegetation,containing gland,stroma and blood vessel.The symptoms of EPs can be asymptomatic polyps,irregular uterine bleeding and sterility.EPs have a low malignant rate.Hysteroscopic resection of EPs is considered to be the most effective and safe as a diagnostic and therapeutic intervention.However,the recurrence rate of uterine polyps after operation is high,Therefore,it is necessary to choose an effective plan to prevent recurrence after operation of EP.Drospirenone and ethinylestradiol tablets II is a new type of compound oral contraceptive.It has less side effects because of its low dose of estrogen,there are few reports on the clinical effect of Dospirenone and ethinylestradiol tablets II on the prevention of recurrence of EPs after operation.In this paper,we studied the clinical effect of Drospirenone and ethinylestradiol tablets II on the prevention of recurrence after hysterectomy in different pathological types of EPs,and to understand its therapeutic effect and side effects,so as to provide clinical experience for prevention of postoperative recurrence of polyps.Methods:We choose 205 patients with endometrial polyps who were treated by hysteroscopic surgery in the Department of Gynecology and Obstetrics of General Hospital of Tianjin Medical Universityfrom September 1,2017 to September 1,2018.The pathological types,age distribution and abnormal menstruation of 205 patients with EPs were statistically analyzed.For functional polyp patients,according to whether the patients have used drugs to prevent the recurrence of polyps after operation,they were divided into observation group(n = 14)and control group(n =15).According to the different drugs used to prevent the recurrence of polyps,patients with non-functional polyps were divided into three groups: Drospirenone and ethinylestradiol tablets II treatment group(n = 21),Dydrogesterone group(n = 23)and control group(n = 21).The improvement of intimal thickness and abnormal menstruation were evaluated.The data results were statistically analyzed by SPSS22.O software,P < 0.05 was statistically significant.Results:(1)Non-functional polyps were the most common(42%)and adenomyomatous polyps were the rarest(3%)among four types.(2)The highest incidence age of functional polyps was 30-39 years old(69%),followed by 20 years old 29 years old(16%),non-functional polyps patients 30 years old 39 years old(58%),followed by 40 years old patients(34%).(3).Most of functional EP were asymptomatic(54%),while the most common manifestations of non-functional EP were irregular uterine bleeding(63%).(4)Adenomyomatous EP mainly occur in postmenopausal patients.(5)The intimal thickness of functional polyposis group was lower than that of control group at 6 months after operation(P < 0.05).There was significant difference between the two groups.(6)Both of the two treatment group were significantly better than that in the control group(P < 0.05).The average thickness of endometrium in Drospirenone and ethinylestradiol tablets II group was lower than that in Dydrogesteronegroup 6 months after operation,but the statistical result was no significant difference,P > 0.05.(7)Of the 35 patients who were treated with Drospirenone and ethinylestradiol tablets II,17 had irregular vaginal bleeding in the first cycle of treatment,and disappeared in the second month.Conclusion:(1)The highighest incidence age of functional polyps and non-functional polyps was between 30 to 39 years old,but the incidence age of functional polyps was younger.(2)Most of the patients with functional polyps were asymptomatic polyps;The most common manifestation of nonfunctional polyps is uterine irregular bleeding.(3)Drospirenone and ethinylestradiol tablets II can be used as a clinical treatment for functional polyps and non-functional EP to prevent recurrence after operation,which is safe,effective,convenient and has few side effects,so it can be used as a clinical method for the prevention of recurrence of functional and non-functional EP.
