| Objective:Through clinical study,the clinical curative effect of Zhang Shi Yiwe.i decoction an Hp-relatecd peptic ulcer patients with spleen-stomach qi deficiency syndrome and its effect on gastrointestinal hormone level were observed,To explore the therapeutic effect and mechanism of Zhangshi Yiwei decoction on Hp-related peptic ulcer of spleen-stomach qi deficiency syndrome,and to provide objective basis for Zhang’s Yiwei decoction in treating peptic ulcer.Methods:From March 2018 to December 2018,64 inpatients and outpatients in the Department of Digestive Department of the affiliated Hospital of Jiangxi University of traditional Chinese Medicine were collected.All the patients met the diagnostic criteria of western medicine for Hp-associated peptic ulcer.According to the diagnosis standard of syndrome of deficiency of spleen and stomach qi,32 cases in treatment group and 32 cases in control group were randomly divided into treatment group(n=32)and control group(n=32).The patients in the treatment group were treated with proton pump inhibitor plus Zhang Shi Yi Wei decoction for 2 months.Detection of Hp in patients with GAS,SS;13C by enzyme-linked immunosorbent assay,according to the principle of diagnosis and treatment of peptic ulcer in the guiding principles of C linical Research of New drugs of traditional Chinese Medicine,the integral of TCM syndrome and the clinical curative effect of patients with peptic ulcer were evaluated.Outcome measures:(1)the correlation between peptic ulcer and clinical data was analyzed by chi-square and t-test,(2)the total score of TCM symptoms before and after treatment was compared between the two groups,and the clinical curative effect was observed.(3)to compare the changes of gastrointestinal hormone level before and after treatment.(4)Gastroscope effect.Results:(1)Analysis of the relationship between peptic ulcer patients and sex:there were 26 females(40.63%),38 males(59.37%),and the ratio between males(1.46%)was 1.46%,which was significantly higher than that of females(P<0-05).(2)Analysis of the relationship between peptic ulcer patients and age:18-33-year-old age group accounted for 42.19%,followed by the middle-aged group,accounting for 29.69%.(3)Analysis of the relationship between peptic ulcer patients and the course of disease:the course of disease was 3 months in 32 cases,the total number was 50.00%,and th,course of disease was more than 6 months in 26 cases,accounting for 40.63%of the total..(4)Analysis of peptic ulcer patients and pathogenic factors:mainly living and working pressure,irregular diet;Work,life pressure(25.00%)>irregular diet(21.88%)>spicy,fat and sweet taste(20.31%)>drinking(15.63%)>cold and cold die:t(10.93%)>smoking(6.25%).(5)T he scores of TCM symptoms:after treatment,there was no significant change in the improvement of mental fatigue and lazy speech between the two groups(P>0.05),while the other TCM symptoms were improved in both groups,and the improvement in the treatment group was better than that in the control group(P<0.05),(8)Clinical efficacy:the total effective rate was 71,88&and the total effective rate was 87.50%.The total effective rate of the control group was 62.50%,and the total effective rate was 78.13%.The clinical curative effect of the treatment group was better than that of the control group(P<0.05).(7)Gastrointestinal hormones:the levels of GAS in the treatment group were significantly lower than those before and after treatment(P<0.05),indicating that the treatment group could significantly reduce the level of GAS.The abnormal GAS level of the patients was within the normal reference range.The level of SS in the treatment group was significantly higher than that before and after treatment(P<0.05),indicating that the SS level in the treatment group was significantly higher than that in the control group(P<0.05).There was no significant difference in the levels of GAS and SS in the control group before and after treatment(P>0.05),indicating that the control group had no significant effect on regulating the abnormal levels of GAS and SS in the control group.Treatment group and pairThe level of GAS after treatment in the exposure group was significantly higher than that in the control group(P<0.05).There was statistical difference in SS level between the two groups after treatment(P<0.05).(8)Hp eradication rate:the total effective rate of Hp was 87.50%in the treatment group and 81.25%in the control group.There was significant difference in the total effective rate between the two groups.(9)comparison of the efficacy of gastroscopy:the total effective rate was 68.75%,87.50%in the treatment group and 62.50%and 78.13%in the control group,respectively.There was significant difference between the two groups.Conclusion:Zhangshi Yiwei decoction is effective in the treatment of Hp-related peptic ulcer,and its mechanism may be related to its regulation of gastrointestinal hormone level and inhibition of Hp.In addition,Zhang’ s Yiwei decoction has obvious advantages in improving symptoms,drug safety and prevention of recurrence. |
