The operating system of operating rooms is an important part of the hospital service system.With the shortage of medical resources in China,improving the management efficiency of the operating room is a vital way to mature the domestic hospital operation management system.Focused on the process of elective surgery patients receiving medical services,try to optimize the management of operating rooms on the basis of data research.Operating room management needs the coordination of other departments,as operating room involves a variety of resources management,including medical staff working time,occupancy of the beds,etc.Therefore,operating room scheduling should fully consider the whole treatment process of patients,and balance all resources allocation strategy.Based on the actual findings of a first grade hospital in Shanghai,this study analyzed its detailed surgical data of orthopedics,cardiology,neurology,thoracic,obstetrics and gynecology surgery from November 2014 to October 2015 and the data of gynecology ward patients' treatment process from January 2013 to December 2015.The study described and analyzed the hospital's utilization efficiency of ward and operating room.The effective description of the surgery duration is foundation of the operating room scheduling model.Due to the complexity of the surgery,the surgery duration has a high degree of uncertainty,which increases the difficulty of scheduling process.So a hypergamma probability distribution model was applied in the fitting of the empirical surgery duration distributions of all surgeries,surgeries from various departments,surgeries of different kinds.The parameters of the fitting model were estimated by Expectation Maximization(EM)algorithm and Genetic Algorithm(GA)respectively.The root mean square error(RMSE)was applied to measure the fitting accuracy,and numerical results showed that the hypergamma probability distribution model was more suitable in the fitting of the surgery duration distributions than the classical probability distributions such as Lognormal,Gamma and Weibull Distributions.The proposed GA was superior to the classical EM algorithm in the parameters' estimation efficiency.Hospital admission control strategy for elective surgery patients will directly affect the service demand of the operating room.Therefore,operation management should consider both admission control strategy and operating room plan.A rolling integer linear programming model was established to decide patient admission control strategy and make operating room plan jointly,and to provide advice for every patient has elective surgery need.The model considers the common benefits of patients and the hospital.The objective function is composed of patient admission waiting costs,surgery waiting costs,operating room overwork costs,and surgery scheduling preferences.The model improves the practical application ability by the process of rolling solution in the cycle of one week.Take the expected service ability of the ward and the operating room as input and get solution,then evaluate the decision by updating the actual ward and operating room utilization.The application in the basic case verifies the optimization efficiency of the operating room scheduling model.The sensitivity analysis explains the influence of every parameter on the model.
