| Objectives To investigate the effects of shifting,rhythm-related gene polymorphisms and their interactions on HUA among steel workers.Methods A total of 7617 steel workers of the HBIS GROUP TANGSTEEL COMPANY’s main production unit who participated in the 2017 annual occupational health examination,were interviewed with a standardized questionnaire and physically examine.After relevant information and blood samples were collected,we use the restricted cubic spline(RCS)model to analyze the dose-response relationship between the duration of night shift and HUA.Then we used the results of RCS model to classify the duration of night shift into three groups,and analyzed the association between night shift work and HUA with other covariate factors in a multivariate logistic regression model.In the subsequent study,484 male HUA patients and 484 male controls were randomly selected from the 7617 patients with complete baseline information with the help of a SAS macro program according to criteria of the study design.The genomic DNA was extracted using the TIANnamp Blood DNA Kit.The target gene fragment is then amplified by polymerase chain reaction(PCR),and the product is digested by restriction endonuclease which helps us to determine the genotype on agarose gel electrophoresis.Finally,generalized multifactor dimensionality reduction method(GMDR)was used to analyze the influence of the interaction between genes and genes,genes and environment on HUA.Results 1 The age-standardized prevalence of hyperuricemia in males and females in the study population was 37.54%and 20.92%,respectively;2 The results of RCS model analysis showed that there was Non-linear dose-response relationship between lifetime night shift duration and hyperuricemia(P<0.001 for overall association test,P<0.001 for nonlinear test),after adjusting for age,gender,education level,monthly income per capita,smoking,drinking,dietary,sleep duration,sleep quality,physical activity,BMI(categorical variable),abnormal liver enzyme metabolism,dyslipidemia,impaired renal function,hypertension,high temperature,noise,dust,and carbon monoxide exposure;3 The multi-factor logistic analysis showed that the main factors affecting hyperuricemia were age,gender,income level,smoking,drinking,night shift,hypertension,dyslipidemia,abnormal liver enzyme metabolism,impaired renal function and diabetes,according to the result of RCS.Compared with never shifters(WSI=0d),short-term shifts(0<WSI≤1676.5d)increased the likelihood of hyperuricemia by 27.5%(OR=1.275,95%CI:1.0911.490)),while the long-term shift(WSI>1676.5d)was not statistically related to HUA(OR=1.037,%95CI:0.8841.217);4The results of stratified analysis showed that the effect of short-term shift on HUA mainly existed in drinking,overweight,lower DASH diet scores,insufficient sleep duration,insomnia and dyslipidemia.Notably,there was an effect modification relationship between diabetes and shift(Pinteraction<0.05);5 The RORA gene rs8034880 polymorphism was associated with HUA significantly,including the recessive model(AA,AG vs GG)(OR=0.71,95%CI:0.530.71)and the additive model(G allele)(OR=0.83,95%CI:0.700.98);6 There is no interaction between RORA(rs8034880)gene and MTRN1A gene(rs2119882),CLOCK gene(rs1801260)and PRE2 gene(rs2304672)based on multiplicative model and additive model;7 MTRN1A gene rs2119882 locus polymorphism combined with night shift work,TC gene carriers showed an increased risk of HUA in short-term shifters(OR=2.01,95%CI:1.133.59)and long-term shifters(OR=1.77,95%CI:1.013.09),but no interaction was found based on the multiplication model.8 The association between the CLOCK gene rs1801260 polymorphism and HUA was not significant,as was the PER2 gene rs2304672 polymorphism and HUA.Conclusions 1 There is a nonlinear dose response relationship between the weighted shift work index(WSI)and HUA;2 Night shift,age,gender,BMI,income level,smoking,drinking,diabetes,hypertension,impaired renal function,dyslipidemia,and abnormal liver enzyme metabolism are the main independent risk factors for HUA;3 Diabetes has an effect modification on the relationship between WSI and HUA;4 RORA gene rs8034880 locus gene polymorphism is associated with male steel worker HUA;5 The MTRN1A gene rs2119882 locus and the CLOCK gene rs1801260 locus gene polymorphism have a joint effect with shifting,but there is no interaction based on the multiplicative model;6 No association was found between the polymorphism in rs2304672 locus of PER2 gene and HUA.Figure6;Table39;Reference 177... |
