| Objective:Clinical analysis of Hashimoto’s thyroiditis(HT)combined with papillary thyroid microcarcinoma(PTMC)and to explore its clinical regularity.Methods:Through retrospective analysisy,95 cases of thyroid micropapillary carcinoma were treated by surgery from January 2018 to December 2018,and were classified according to whether Hashimoto’s thyroiditis was combined: The thyroid micropapillary carcinoma with Hashimoto’s thyroiditis have 30 cases,named the study group,simple thyroid micropapillary carcinoma as a control group,have 65 cases,to compare the difference of two groups including gender,age,tumor maximum diameter,preoperative thyroid stimulating hormone level,the effect of the thyroid antibody level on the central lymph node metastasis,the detection rate of lymph nodes,and the value of preoperative thyroid ultrasound assessment of cervical lymph node metastasis.Results:1.There were 24 female patients in the PTMC group with HT,accounting for 80% of the group;39 female patients in the PTMC group,accounting for 60% of the group,P<0.044,the difference was statistically significant;2.The combination of HT PTMC had no significant difference in tumor maximum diameter,age,TSH level and PTMC alone.3.The concentration of thyroglobulin antibody and peroxidase antibody had no effect on central lymph node metastasis.4.There was no significant difference in lymph node detection rate and central lymph node metastasis rate between PTMC group and PTMC group.5.5.Thyroid ultrasound indicated that the sensitivity of positive cervical lymph nodes was higher than that of central lymph nodes;Compared with the simple PTMC group,the PTMC group combined with HT showed no statistical difference in the sensitivity and specificity of ultrasound to the central region lymph nodes.Conclusion:Hashimoto’s thyroiditis with thyroid micropapillary carcinoma occurs in women,and there is no difference in tumor diameter,preoperative TSH level and age compared with PTMC alone.Thyroid antibody levels had no significant effect on central lymph node metastasis.Combined HT did not affect the detection rate of PTMC lymph nodes and central lymph node metastasis rate.The sensitivity of lymph node positive in central region was low,and the value of lymph node metastasis in cervical group was high. |
