Peri-implant disease refers to chronic inflammation of the tissue surrounding the implant.Some problems such as bone resorption around the implant,which is one of the most common complications after implantation,may happen in some cases.There are many susceptibility factors causing such diseases around implants.For example,cement-retained may have some effects on the peri-implant tissue.In serious cases,it can even cause peri-implantitis.Objective: At present,two bonding methods are used commonly in clinic,one is improved bonding fixation method,which is first cement crown to the abutment,then fixes this complex by screw to implant in oral.Another is bonding fixation method,which is directly cement crown to the abutment in oral.Studies have shown that the health of improved bonding fixation method is better than the bonding fixation method.The most of the study results are supplied from clinic observation,they should be confirmed and enriched by experimental study.Through measuring and comparing the level of IL-6 and TNF-α expression in gingival crevicular fluid between the two different bonding methods,to discuss effect of cement-retained in peri-implant tissue,and provide a valuable theoretical basis for the prevention of inflammation around implant in clinic.Methods: There were a total of 38patients(23 male,15 female,average age: 43.4),which undergoing ITI implant surgery and restoration in The Third hospital of Hebei Medical University from December 2016 to November 2017.And all porcelain crowns restored for each implant were already 6-9 months,were selected as the research object.All the patients are divided into two groups according to the above bonding methods: improved bonding fixation group and bonding fixation group,each group has 19 patients with 19 crowns.Examine the plaque index and gingival sulcus bleeding index of the each implant and corresponding tooth.Finally,the gingival crevicular fluid was extracted around the implant and the corresponding tooth by filter note,and the weight of the gingival crevicular fluid was measured by electronic balance.After the collection is completed,the concentration of IL-6 and TNF-α in PICF/GCF is detected by ELISA Kit in the laboratory.Using SPSS20.0 to analyze the obtained data statistically,the difference of P <0.05 was statistically significant.Results: There was no statistical difference between the two group of the weight of the gingival crevicular fluid and the IL-6 and TNF-α concentration of the corresponding tooth.The weight of gingival crevicular fluid in bonding fixation group was higher than that of improved bonding group,and the difference was statistically significant(P<0.05).The concentration of TNF-α in the gingival crevicular fluid of the bonding fixation group was higher than that of the improved bonding fixation group,and the difference was statistically significant(P<0.05),The concentration of IL-6 in the gingival crevicular fluid of the bonding fixation group was higher than that of the improved bonding fixation group,and the difference was statistically significant(P<0.05).There was a positive correlation between the weight of gingival crevicular fluid and TNF-α and IL-6,and the correlation coefficient R was 0.342 and 0.248,P<0.05.Under the two fixation methods,there was no statistical difference between Plaque Index and Sulcus Bleeding Index.Conclusion: The concentration of IL-6,TNF-α and the weight of gingival crevicular fluid in natural teeth were not statistically different between the two groups,indicating that there was no difference in periodontal tissue between the two groups,the concentration of IL-6 and TNF-α in the gingival crevicular fluid and the weight of the gingival crevicular fluid around the implant are affected by the bonding methods.In this experiment study,the weight of gingival crevicular fluid around the implant prosthesis with improved bonding fixation is lower than that of the bonding fixation.When we use improved fixation method,the excess adhesives is effectively avoided and less inflammation would happen,which is beneficial to improve periodontal health.In the study of the levels of TNF-α and IL-6,we came to the result that the levels of the two cytokines detected around the prosthesis of the improved bonding fixation were lower.This suggests that there are fewer stimuli in the gingival sulcus,resulting in lower levels of cytokines secreted by the body.According to the result,we suggest selecting improved bonding fixation method as a ideal way in clinic.Further research on IL-6 and TNF-α in gingival crevicular fluid around implant may works in the prevention and early treatment of peri-implant diseases.
