Object:Based on the perspective of supply and demand,this study analyzes the differences in the implementation effects of urban and rural basic public health service projects in Taiyuan City,and provides a basis for policy research on urban and rural basic public health services in Taiyuan.Mrthods:Research methods include literature research method,multi-stage random Sampling method,expert consultation method,comprehensive evaluation method and questionnaire survey method.Based on the literature research on basic public health services in urban and rural areas,we will go deep into the 10 areas of Taiyuan City and use the multi-stage random sampling method to extract 2 primary health Care institutions(1 community health service center and 1 township health center)in each district.On-site performance appraisal,the appraisal project score is determined by the expert consultation method,the appraisal results are expressed by the completion rate or scoring rate,and the comprehensive evaluation method is used to analyze the effect of each project.With the questionnaire survey results,the difference analysis between supply and demand is carried out.Results:By analyzing 12 primary indicators and 20 secondary indicators of the basic public health service project in this study,compared with the 2017 national basic public health service task standards,the results are as follows:the overall implementation effect of urban and rural basic public health service projects in Taiyuan City in 2017 Preferably,the city(GI=80.03)is better than the rural area(GI=76.5).Supply side:There is no difference in the management items of urban and rural patients with severe mental disorders(?~2=2.99 P=0.084).In other projects,urban grassroots medical and health institutions are better than rural areas,including health file establishment and use,health education,prevention.Vaccination,child health management,maternal health management,health management for the elderly,management of Chinese medicine health,reporting and handling of infectious diseases and public health emergencies,and co-ordination of health and family planning supervision.Rural primary health care institutions are better than urban health management programs including chronic disease patients and health management programs for tuberculosis patients.The perspective of the demand side:urban and rural residents in the infectious disease and public health incident report and treatment projects and health care program supervision and management services project self-satisfaction to receive services,the remaining projects,the proportion of urban residents receiving services is higher than rural areas including Establishment and use of health records(City:78.31%Township 77.7%),Vaccination(City:74.4%Township:69.88%),Maternal Health Management(City:49.8%Township:47.79%),Health Management for the Elderly(City:87.55%Township:74.3%),Chinese Medicine Health Management(City:7.23%Township:1.4%).Rural residents receiving services with a higher proportion of services including health education,child health management(City:64.66%Township:65.86%),health management of chronic disease patients(City:63.05%Township:81.93%),management of patients with severe mental disorders(City:3.42%Township:4.29%),health management of tuberculosis patients(City:3.28%Township:7.23%).Conclusion:The implementation effect of urban and rural basic public health service projects in Taiyuan City is different and the implementation effect of urban grassroots medical and health institutions is better than that in rural areas.Rural areas can increase the welfare of residents receiving grassroots services,strengthen the construction of grassroots public health teams,and improve the grassroots level.Facilities,special rooms,increase the construction of information system modules for primary health care institutions,and reduce the difference between the implementation of basic public health service projects in urban primary health care institutions.
