| Objective:To study the histopathological features of synchronous bilateral breast cancer and explore the related factors affecting prognosis.Methods:The clinical data of 151 patients with synchronous bilateral breast cancer diagnosed in Peking Union Medical College Hospital from January 2008 to December 2018 were collected,including age,gender,family history of cancer,family history of breast cancer,prognosis and so on.According to whether both breasts are invasive cancer,they were divided into three groups,including synchronous bilateral invasive breast cancer,synchronous bilateral breast cancer in situ,one breast is invasive cancer and the other breast is cancer in situ.Because synchronous bilateral invasive breast cancer has the largest number and the worst prognosis,this study focused on the histopathological characteristics and immunohistochemical features of the tumors,as well as the consistency of expression on both sides,and further explored the pathological factors which affecting the prognosis.Results:Synchronous bilateral breast cancer accounted for 1.23%of all breast cancer patients in the same period,mainly in women,and the proportion of breast cancer family history was 14.6%.In synchronous bilateral invasive breast cancer,the main histological type is non-special type,the main histological grade is moderate differentiation,the main TNM stage is in stage I,the main tumor size is T1,the main regional lymph node is NO,the main molecular subtypes are Luminal A and Luminal B,ER and PR are mainly positive expression,HER2 is mainly negative expression.The consistency of histological type and histological grade was 80.2%and 70.8%,respectively.The consistency of the tumor size T stage and the regional lymph nodes N stage was 58.5%and 55.7%,respectively.The consistency of the molecular subtype was 48.1%.The consistency of the ER,PR,HER2 and Ki-67 expression was 71.7%,66.0%,76.4%and 49.1%,respectively.There was a significant correlation between the expression of hormone receptor status and prognosis in synchronous bilateral invasive breast cancer.(P=0.000).Conclusions:The prognosis of synchronous bilateral invasive breast cancer was the worst in synchronous bilateral breast cancer,while the histopathological and immunohistochemical characteristics of left and right breast cancer were more consistent.The expression of hormone receptor status in bilateral breast cancer was significantly different from that in prognosis,that is,patients with ER-discordant tumors had better prognosis than patients with ER concordant-negative tumors,whereas patients with ER concordant-positive tumors had the best prognosis.It is recommended that we should pay attention to the detection of hormone receptors in bilateral invasive breast cancer in daily pathological diagnosis. |
