Objective:To study the clinicopathological characteristics of primary bilateral breast cancer(BPBC)and analyze its prognostic factorsMethods:Through retrospective analysis,we collected 116 BPBC patients diagnosed and treated in the First Affiliated Hospital of Shihezi University School of Medicine and Affiliated Tumor Hospital of Xinjiang Medical University from June 2013 to June 2020,and randomly selected patients with unilateral breast cancer(UBC)during the same period 232 cases,collected general clinical information and histopathological data of patients,analyzed by chi-square test,followed up BPBC patients,and analyzed the clinicopathological characteristics and prognosis of BPBC using Kaplan-Meier method and Cox proportional hazard regression model Influencing factors.Results:1.Cohen’s Kappa statistical coefficients were used to analyze the expression of ER,PR,Her-2,and Ki67 in bilateral primary tumors of BPBC with strong consistency(P<0.001).Using Cox to analyze the relationship between related factors and patient prognosis,and comparing with related factors,it can be concluded that the risk of death in patients with family history is higher than that in patients without family history,HR=2.848(95% CI: 1.022-7.939).The death risk of patients with metachronous bilateral primary breast cancer(m BPBC)was lower than that of patients with simultaneous bilateral primary breast cancer(s BPBC),HR=0.24(95% CI: 0.069-0.838).2.The family history of BPBC patients(P<0.05),ER positive(P<0.05),PR positive(P<0.05),and high Her-2 expression(P<0.05)of the first tumor immunohistochemical results were all higher than those of UBC.The difference between the subjects was statistically significant(P<0.05).3.The ER positive(P<0.05)and the high expression of Her-2(P<0.05)in the first tumor of s BPBC were higher than that of m BPBC,and the difference was statistically significant(P<0.05).The stage of the second tumor in m BPBC was lower than that of the first tumor(P<0.05).The difference was statistically significant(P<0.05).Conclusion:1.In the bilateral tumors of BPBC,the expressions of ER,PR,Her-2,and Ki67 have strong consistency.Family history and simultaneous occurrence of bilateral tumors are risk factors related to the prognosis of BPBC.2.The risk factors of BPBC include family history,ER positive,PR positive,Her-2 positive.3.Compared with m BPBC,s BPBC has higher ER-positive and Her-2 positive expression.ER-positive and Her-2-positive are risk factors for s BPBC.The first tumor of m BPBC has a higher stage than the second tumor.
