1.Study on the Distribution Characteristics of PCOS Syndrome(ovulation disorder)Objective:In this study,PCOS patients(hereinafter referred to as PCOS)with normal female hormone and fasting insulin but sparse ovulation or non-ovulation were taken as the research objects.The distribution characteristics of PCOS syndrome elements and types were preliminarily explored,and the main symptoms of PCOS syndrome were refined to provide reference for clinical syndrome differentiation.Research methods:From September 2016 to June 2018,120 patients with PCOS who met the inclusion and exclusion criteria were selected from the gynecological clinic of Guang’anmen Hospital,Chinese Academy of Traditional Chinese Medicine.Collect the data of four TCM diagnoses and fill in the "PCOS(ovulation disorder)syndrome distribution observation form" regularly.SPSS22.0 statistical software was used to carry out frequency statistics,cluster analysis,principal component analysis and factor analysis of the data,and summarized PCOS(ovulation disorder)syndrome elements,syndrome type distribution characteristics and syndrome main symptoms.Results:1.A total of 125 clinical observation tables were issued and 120 were actually recovered with a recovery rate of 96.00%.2.The average age of the patients was(27.31+4.44)years;the period of irregular menstruation was 3-5 years,43 cases,accounting for 35.83%;66 married women,accounting for 55.00%;50 cases with pregnancy requirements,accounting for 41.67%;52 cases(43.33%)had undergraduate education,followed by 31 cases(25.83%)with junior college education,22 cases(18.33%)with senior high school education,11 cases(9.17%)with master’s degree and 4 cases with doctoral degree.(3.33%).The nature of work was mainly mental workers.72 cases(60.00%),followed by free workers and physical workers,29 cases(24.17%)and 19 cases(15.83%)respectively.3.Frequency statistics showed that PCOS(ovulation disorder)syndrome elements mainly include:kidney deficiency,liver depression,phlegm and dampness,blood stasis,Qi stagnation,spleen deficiency,Yang deficiency.The main factors of disease location syndrome are kidney,liver and spleen;the main factors of disease syndrome are Qi stagnation,phlegm-dampness,blood stasis and Yang deficiency,among which kidney-dominated deficiency symptoms occur more frequently.4.Cluster analysis showed that PCOS syndrome types included kidney deficiency and liver depression(42.50%),Phlegm and blood stasis(22.50%),liver depression and spleen deficiency(18.33%)and qi stagnation and blood stasis(16.67%).5.Factor analysis showed that the main symptoms of PCOS(ovulation disorder)syndrome of kidney deficiency and liver depression were postmenstrual malfunction,lumbar acid,lumbar cooling,reddish menstruation,alopecia,irritability,pre-menstrual breast pain,abdominal pain and pulse strings;the main symptoms of Spleen-Kidney Deficiency and phlegm-stasis syndrome were fatigue and fatigue,vulva dampness.excessive leucorrhea,pulse slippery,postmenstrual malfunction,fat tongue,tongue with blood stasis,menstruation.There are blood clots;the main symptoms of liver depression and spleen deficiency syndrome are postmenstrual malfunction,yellowish complexion,fatigue,fatigue,fat tongue,sticky stool,irritability,pulse string;the main symptoms of kidney deficiency and blood stasis syndrome are postmenstrual malfunction,dark red menstruation,irritability,pre-menstrual breast swelling and pain,abdominal pain,blood clots,tongue stasis,pulse slippery.Conclusions:1.PCOS(ovulation disorder)is characterized by kidney,liver and spleen.The main factors of the disease type syndrome are Qi stagnation,phlegm dampness and blood stasis.2.The basic TCM syndromes of PCOS(ovulation disorder)include kidney deficiency and liver depression.Phlegm and blood stasis,liver depression and spleen deficiency,qi stagnation and blood stasis.2 Clinical Observation on Treatment of PCOS(Ovulation Disorder)by Kidney-tonifying and Liver-regulating TherapyObjective:To summarize the efficacy and safety of Kidney-Tonifying and Liver-regulating therapy for PCOS patients with Kidney-Deficiency and liver-depression syndrome(ovulation disorder).Research method:In this study,90 patients with PCOS(ovulation disorder)of kidney deficiency and liver depression syndrome were collected retrospectively from September 2016 to December 2018 in the gynecological clinic of Guang’anmen Hospital,Chinese Academy of Traditional Chinese Medicine.The clinical data of the first,second,third.fourth,fifth and sixth cycles of treatment were collected.The number of ovulation and pregnancy were counted,the ovulation rate and pregnancy rate were calculated,and the scores of TCM clinical symptoms,menstrual cycle,menstrual period,menstrual volume,SAS score,SDS score,liver and kidney function were compared before and after treatment.SPSS22.0 statistical software was used to analyze the data and evaluate the efficacy and safety of Bushen Tiaogan method in treating PCOS(ovulation disorder)of kidney deficiency and liver depression syndrome.Results:1.A total of 90 cases were collected.The average age of 90 patients was(27.44+4.47)years old;31 cases(34.44%)had irregular menstruation for 3-5 years,followed by more than 10 years,26 cases(28.89%),followed by 6-10 years and less than 3 years,19 cases(21.11%)and 14 cases(15.56%)respectively;the majority of married persons were married,80 cases(88.89%)and 75 cases had pregnancy needs.2.After treatment,the ovulation rate gradually increased from the first cycle to the sixth cycle,and reached 72.06%in the sixth cycle.During the 6th cycle of treatment.35 of 75 patients with pregnancy requirements were pregnant,with a cumulative pregnancy rate of 46.67%.3.The menstrual cycle scores in the 3rd,4th,5th and 6th cycles,5th and 6th cycles,and the menstrual volume scores in the 6th cycle were significantly lower than those before treatment(all P<0.05).4.The scores of TCM clinical symptoms in the 3rd,4th,5th and 6th cycles of treatment were significantly lower than those before treatment(all P<0.05).5.After treatment,SAS and SDS scores were significantly lower than those before treatment.S AS scores were significantly lower from the 2nd to 6th cycles of treatment than those before treatment(all P<0.01);SDS scores were significantly lower from the 1st to 6th cycles of treatment than before treatment(all P<0.05).Conclusions:1.The method of tonifying kidney and regulating liver can obviously improve the menstrual cycle,menstrual period and menstrual volume of patients with PCOS of kidney deficiency and liver depression syndrome,help ovulation and pregnancy,significantly alleviate clinical symptoms and patients’ anxiety,depression and other adverse emotions,and has a certain effect.2.The medicine of tonifying kidney and regulating liver has no obvious influence on the function of liver and kidney in treating PCOS of kidney deficiency and liver depression syndrome,and has certain safety. |
