ObjectiveTo explore the application effect of Omaha system in patients with indwelling double J stent? MethodA total of 249 patients with double J stent indwelling in urology department from May to August 2018 in Hengyang City of Hunan Province were selected and divided into intervention group(124 cases)and control group(125 cases)by stratified random grouping method. The patients were divided into two groups: intervention group(n = 124)and control group(n = 125).Routine nursing was given in the control group, transitional care based on Omaha system was given in the intervention group on the basis of routine nursing, and Omaha evaluation system was used to evaluate the two groups of patients in the field of environment and social psychology, and the patients in the intervention group received continuous nursing based on Omaha system. The scores of cognition, behavior and condition were compared before and after intervention in physiological domain and health-related behavioral field. The patients in both groups were also compared during the period of indwelling double J tube. The incidence of the disease was compared.Results1.There was no significant difference between the intervention group and the control group in the field of environment, social psychology, physiology and health-related behavior before intervention(P > 0.05).2.Before intervention,7 days,15 days and 1 month of intervention, the scores of Omaha evaluation system were evaluated in the intervention group and the control group. After repeated measurement of variance analysis, the results showed that the two groups of patients were in the environment, social psychology, and so on.There were significant differences in time effect,inter-group effect,time-to-group interaction effect between 14 nursing problem scores in physiological and health-related behavioral fields(P < 0.05).It is shown that the score changes with time without considering the intervention factors, and the score will change with the intervention measures without considering the time factors.3.After intervention, the total incidence of double J tube related complications was 55% in the intervention group and 78% in the control group(P < 0.05),and there was a significant difference between the two groups(P < 0.05),in which the intervention group had a significant difference in hematuria and infection, while in the control group, the incidence of double J stent related complications was 78%.The incidence of bladder irritation sign and stagnation was lower than that of the control group(P < 0.05).In the intervention group, the incidence of wall-attached stone or scaling and residual stone was lower than that of the control group, but there was no significant difference between the intervention group and the control group(P > 0.05).Conclusion1.The application of Omaha system in patients with double J tube indwelling urolithiasis after operation is feasible.2.The transitional care based on Omaha system can effectively improve the scores of cognition, behavior and condition of patients with indwelling double J tube in the fields of environment, social psychology, physiology and health-related behavior.3.Transitional care based on Omaha system can reduce the incidence of complications in patients with double J-tube indwelling.
