Application And Evaluation Of The Cardiac Rehabilitation Transitional Care Based On Omaha System In Coronary Heart Disease Patients With Percutaneous Coronary Interventions

ObjectiveBased on Omaha System,the cardiac rehabilitation transitional care in coronary heart disease patients with Percutaneous Coronary Interventions was established,in order to analyze main nursing problems and interventions during the rehabilitation period,and to explore the effects of quality of life on coronary heart disease patients with Percutaneous Coronary Interventions.MethodsUsing the convenience sampling method,we selected 100 coronary heart disease patients with Percutaneous Coronary Interventions for the research.By applying the method of random number table,the patients were randomly divided into two groups,50 cases of control group and intervention group of 50 cases.Finally,each group had 47 patients.The Control group patients received routine nursing care,including regular hospital inpatient care,guidance and follow-up after discharge.The Intervention group patients were given the cardiac rehabilitation transitional care based on Omaha System.Finally,we use Likert 5 scale evaluate and score to compare the effect of problems,and the differences of knowledge,behavior and status of the patients in the intervention group.At the same time,the life quality of the two grops was evaluated by using the Quality of Life measured Scale.All data were analyzed by SPSS22.0.Results1.The Assessment scale based on the Omaha System was constructed by Delphi Method.The intervention program was constructed based on the Omaha System.A evaluation example of nursing outcomes was developed and scored from knowledge,behavior and status.2.There were 534 nursing problems of patients in the intervention group during the hospitalization,and they were problems of health-related behaviors(47.2%),physiology(27.5%),psychology(21.0%)and environment(4.3%).After discharge,there were 520 nursing problems of patients in the intervention group,and they were problems of health-related behaviors(54.4%),psychology(22.7%),physiology(16.2%),and environment(6.7%).3.During the period of hospitalization,the intervention group patients had 8 nursing problems whose incidences were over 30%,and there are respectively physical activity(85.1%),nutrition(83.0%),circulation(59.6%),substance abuse(55.3%),mental health(55.3%),pain(46.8%),personal care(40.4%)and breathing(38.3%).After discharge,the intervention group patients had 9 nursing problems whose incidences were over 30%,and there are respectively physical activity(68.1%),nutrition(63.8%),mental health(51.1%),circulation(44.7%),pain(40.4%),Substance abuse(34.0%),personal care(31.9%),drug treatment project(31.9%)and breathing(38.3%).4.During the period of hospitalization,the intervention group patients had 8 nursing problems whose incidences were over 30%.In addition to the status outcome score of the problem of mental health,there were significant differences in other issues of knowledge,behavior and status outcome scores(P < 0.05).After discharge,the intervention group patients had 9 nursing problems whose incidences were over 30%.In addition to the behavior and the status outcome score of personal care,there were significant differences in other issues of knowledge,behavior and status outcome scores(P < 0.05).5.In the intervention group,all the scores of life quality of the patients including symptoms,physical function,psychological social function,cognitive function,adverse reactions and satisfaction were significantly greater than before the intervention(P<0.05).And after intervention,all the scores of life quality of the intervention group patients including symptoms,physical function,psychological social function,cognitive function,adverse reactions and satisfaction were significantly greater than the control group(P<0.05).Conclusions1.The cardiac rehabilitation transitional care in coronary heart disease patients with Percutaneous Coronary Interventions which is based on Omaha System can comprehensively assess nursing problems from the aspects of physical,psychological,environmental,and behavior.Finally,the transitional care can improve the outcome of patients from knowledge,behavior and status through the implenmentation of specific nursing intervention.2.The cardiac rehabilitation transitional care in coronary heart disease patients with Percutaneous Coronary Interventions which is based on Omaha System can improve patient' s quality of life.3.The Omaha System could be used in the cardiac rehabilitation nursing for coronary heart disease patients with Percutaneous Coronary Interventions.
