| Objective:To investigate the characteristics of refractory epilepsy in young children and related factors of surgical treatmentMethods:In this study,retrospective analysis was conducted on children with refractory epilepsy who were less than 3 years old and received surgical treatment in the neuromedical center of the affiliated hospital of Xiamen university from January 2004 to December 2017.Among them,24 cases were male,and 11 female respectively.To summarize the general conditions,preoperative examinations and operation conditions.Postoperative pathology,APOS,prognosis were summarized and analyzed,comparing the different ages,different pathology and prognosis of disease characteristics,investigate young children with epilepsy surgery prognosis.Results:Age:In the infant group,there were 5 cases of postoperative APOS,8 cases of postoperative complications,and 7(70.0%)cases of prognosis with Engel I.In the infant group,16 patients had postoperative APOS,18 had postoperative complications,and 13(52.0%)patients had Engel I.Pathology:20(57.14%)cases of cortical dysplasia.Long-term postoperative follow-up of MCD group showed that 10 cases were Engel I,9 were non-grade I Engel,and 1 case died.There were 10 cases of Engel I in the non-MCD group and 5 cases of grade I Engel in the non-MCD group.Others:postoperative complications occurred in 25 cases and APOS in 21 casesConclusion:1.The younger the children are and the shorter the course of disease,the better the effect of surgical intervention is.2.Among the pathogenesis,MCD is a common pathogenesis in young children,of which FCD is the main part.Children with MCD are prone to drug resistant epilepsy and psychomotor retardation,and tend to have poor prognosis.3.Postoperative complications are more common in young children,and children with APOS are more prone to poor prognosis than those without APOS. |
