Advance In Study Methods Of Surgical Treatment For Intractable Epilepsy

Objective:To analyze the clinical features of intractable epilepsy(IE),to study the methods of exact localizations of epilepsy focus and surgical treatment of intractable epilepsy,and provide surgical results and experience.Methods:A retrospective analysis was made of 126 patients with intractable epilepsy. All of them were presurgical evaluated and localized by all kinds of means,and performed with the guidance of intraoperative electrocorticography.Under monitor of intraoperative electrocorticography,84 cases received epileptogenic focus resection,11 cases underwent epileptogenic focus resection combined with amygdalohippocampotomy, 3 cases received corpus callosotomy,3 cases receives hemispherectomy,7 cases received mutliple subpial transactions(MST),3 cases received bipolar coagulation,15 cases underwent epileptogenic focus resection combined with bipolar coagulation.The postoperative outcomes were classified by improved Engel's classification.Result:After operation,the seizures frequency and take AEDs were signigicantly fewer than those befor operation.The postoperative outcomes express:94 cases were gradeâ… ,18 cases were gradeâ…¡,13 cases were gradeâ…¢.Conclusion:For intractable,the presurgical evaluation to localize epileptogenic focus has a important value.The combined operations with ECoG monitoring can obtain favorable outcome.The present review supports the position that early and effective epilepsy surgery may not only render the patient with intractable partial epilepsy seizure-free,but also allow the individual to become a participating and productive member of society.
