[Objective]To explore the early screening method of upper gastrointestinal cancer under the new model of hierarchical medical system,and improve the early screening and diagnosis rate of upper gastrointestinal cancer.[Methods]According to screen for esophageal cancer、gastric cancer screening table、stomach function test method for 25 during 2016 to 2019 in Xiamen city community and patients with Xiamen university affiliated hospital of Zhongshan medical center for preliminary assessment,the assessment result for high-risk groups for electronic gastroscopy and biopsy when necessary,according to the result of gastroscopy were analyzed.[Results]Through the evaluation of esophageal cancer screening table,a total of 1013 patients were finally identified as high-risk groups of esophageal cancer,including 463 males and 550 females,with an average age of 49.68±13.15 years.Through the evaluation of gastric cancer screening table、gastric function screening table,1852 patients were identified as high-risk groups for gastric cancer,including 1090 males and 762 females,with an average age of 49.39±10.55 years.In terms of esophageal lesions,11 cases of esophageal cancer were detected,with a detection rate of 1.09%,including 8 cases of early esophageal cancer and 3 cases of advanced esophageal cancer.Precancerous lesions including Barrett’s esophagus and atypical hyperplasia were detected in 18 cases,with a detection rate of 1.78%.The detection rate of reflux esophagitis was 13.13%.In terms of gastroduodenal lesions,7 cases of gastric cancer were detected,with a detection rate of 0.38%,including 6 cases of early gastric cancer and 1 case of advanced gastric cancer.Precancerous lesions were detected in 201 cases,with a detection rate of 10.85%,including 143 cases of atrophic gastritis,76 cases of intestinal metaplasia,and 44 cases of atypical hyperplasia.There were 562 cases of erosive gastritis,and the detection rate was 30.35%.Among them,76 cases of erosive gastritis with atrophy,60 cases of erosive gastritis with intestinal metaplasia,and 27 cases of erosive gastritis with dysplasia.240 cases of ulcer were screened out,and the detection rate was 12.96%,including 162 cases of duodenal ulcer,60 cases of gastric ulcer and 18 cases of complex ulcer.[Conclusion]The appellate screening method can improve the diagnosis rate of esophageal cancer,gastric cancer and upper gastrointestinal precancerous lesions.
