Objective:Radiation-induced mucositis is the most common acute side effect during the radiotherapy of head and neck tumor. This study mainly discussed the efficacy of immunoglobulin antibacterial solution for radiation-induced oral mucositis of head and neck tumor.Methods:Forty-eight patients from Dec 2016 to Jan 2019 with malignant tumors of head and neck(M 29,F 19,36-77 years old, median age is 52.5 years old)who received radiotherapy for the first time in the department of radiotherapy,in Cheng gong Hospital affiliated to Xiamen University were selected. There were 27 patients with nasopharyngeal carcinoma,8 patients with of tongue cancer,7 patients with of hypopharyngeal carcinoma and 6 patients with of laryngeal carcinoma. The patients were randomly divided into two groups:experimental group(n=24)and control group(n=24).There was no significant difference in general data between the two groups(P>0.05).After the patients with head and neck tumors suffered from oral mucositis during radiotherapy, the patiens in experimental group were treated with immunoglobulin antibacterial solution(component:yolk antibody IgY)oral spray,4-6 times a day.Control group:common mouthwash(Gentamicin injection 16000 units, dexamethasone injection 10mg,lidocaine injection 10ml,mixed with 0.9%NS 250ml)took 20ml 3 times a day.All of the patiens observed the same strict routine oral careduring radiotherapy. The degree and recovery time of oral mucosa injury in two groups during radiotherapy were recorded and analyzed statistically to compare the difference of oral mucosal injury between the two groups. Results:Before treatment, there were 14 patients with radioactive oral mucositis grade Ⅱ,the incidence of which was 58.3%In the experimental group, there were 14 patients with radioactive oral mucositis grade Ⅱ,the incidence of which was 58.3%In the control group. There was no distinct difference in oral mucositis level between the two groups(P=0.310).At the 15 times of radiotherapy, the mucosal injury in both groups was mainly grade I,the incidence of grade Ⅰ to grade Ⅳ in the experimental group was 54.2%,16.7%,8.3%,0;and that of the control group was 45.8%,29.2%,12.5%,0;The incidence of total mucosal injury was 79.2%in the experimental group and 87.5%in the control group, and the difference between the two groups was statistically significant(P<0.05).At the end of radiotherapy, the incidence of radiation-induced oral mucosal injury was 20.8%in the experimental group and 33.3%in the blank group. There was a distinct difference between the two groups(P<0.05).The recovery time were(7.42±4.5)days in the experimental group and(15.73±15.5)days in the control group,There was a significant difference of recovery time between the two groups(P<0.05).Conclusion:During the radiotherapy of head and neck tumor, immunoglobulin antibacterial solution can reduce the degree of mucosal injury, shorten the improvement and recovery time of radiation-induced oral mucositis and relieve the uncomfortable feeling of the patient,so as to ensure the smooth progress of radiotherapy. |
