ObjectiveThe purpose of this study was to realize the current situation and analyze influencing factors of childbirth expectation,experience and outcome indicators of primipara.Explored the effect of the birth plan based on the empowerment theory on the childbirth expectation,childbirth experience and the childbirth outcome of the primipara.Methods(1)From July to December in 2017,260 primipara in three tertiary hospitals were asked to answer the following questionnaire: “the general information questionnaire”,“childbirth expectation questionnaire”,“childbirth experience questionnaire” to realize the current situation and analyze influencing factors of childbirth expectation and childbirth experience of primipara.(2)The random sampling method was adopted to extract 88 primipara in three tertiary hospitals of Hengyang city.The primipara were evenly and randomly divided into the control group(n=44)and the intervention group(n=44).In the control group,The primipara were given the routine obsterical service including antenatal examination,antenatal education,midwife clinic counsel and childbirth measures.The primipara of intervention group not only accepted the routine obstetrical service,but also made and carry out the delivery plan according to the principle of empowerment theory with the guide of midwife.Then,compared the two group primiparas’ childbirth expectation scores,childbirth experience scores and delivery outcome indicators.(3)The data were analyzed using Statistical Product and Service Solutions(SPSS18.0).Mean,standard deviation and composition ratio were used for statistical description,t-test,one-way analysis of variance,rank sum test and multiple linear regression analysis were used for statistical analysis.Results(1)The score of childbirth expectation of primipara was(117.74±30.16),its dimensions scores were respectively safe and comfortable medical environment(41.48±10.71),spouse support(19.49±6.10),control and participation(28.31±7.77),labor pain expectation(15.84±4.44),medical support(12.58±3.62).(2)The results of single factor analysis: Four factors had statistically significant effect on the childbirth expectation of primipara(P <0.05).Multiple linear regression analysis showed that age,whether planned pregnancy,the number of pregnant school education were the main influencing factors of the primipara childbirth expectation.(3)The score of childbirth experience of primipara was(60.93±16.746),each dimension score were own capability(19.00±5.713),participation(12.04±3.894),professional support(14.37±5.445),security perception(15.52±3.793).(4)The results of single factor analysis: Five factors had statistically significant effect on the childbirth experience of primipara(P <0.05).Multiple linear regression analysis showed that marital relations,total labor duration and postpartum haemorrhage amount were the main influencing factors of the primipara childbirth experience.(5)Before the intervention,the general information data and childbirth expectation of the two groups primipara were compared,the difference was not statistically significant(P>0.05).(6)When admission,the childbirth expectation of the two groups were compared,the scores of childbirth expectation of the test group and the control group were respectively(113.95±9.36)(107.39±9.47).The scores of childbirth expectation in the test group were higher than those in the control group(P<0.05).(7)Before and after the intervention,the score of childbirth expectation in the control group were respectively(105.40±8.25)(107.39±9.47),there was no significant difference between two groups(P>0.05).The score of childbirth expectation in the test group were respectively(107.25±4.254)(113.95±9.36),there was significant difference(P<0.05).(8)After the childbirth,the score of the childbirth experience of the test group and the control group were respectively(73.80±3.957)(62.88±4.72),the differences of the two groups were significant(P<0.05).(9)After the childbirth,the two groups of mothers’ birth childbirth outcome indicators were compared.The total labor duration,perineum integrity and delivery mode of the intervention group were better than those of control group(P < 0.05).There was no significance difference between two groups of postpartum haemorrhage amount,neonatal 1 min Apgar score,neonatal feeding mode during hospitalization(P > 0.05).Conclusions(1)The childbirth expectation of primipara was at a medium level.(2)Age,whether planned pregnancy,and the number of pregnant school education were the main influencing factors of the primipara childbirth expectation.(3)The childbirth experience of primipara was at a medium level.(4)Marital relations,total labor duration and postpartum haemorrhage amount were the main influencing factors of the primipara childbirth experience.(5)The birth plan based on the empowerment theory can improve the level of childbirth expectation,childbirth experience,improve the outcome of delivery,so that it can improve the satisfaction of the primipara,promote maternal and child health.
