| Objective:Clinical observation of chronic superficial gastritis treated by fire needle plus acupoint catgut embedding is performed to comprehensively evaluate the clinical efficacy of this method in the treatment of CSG.Methods:58 outpatients and inpatients of the 58th Shanxi Provincial College of Traditional Chinese Medicine and Chinese Medicine Hospital of Fangshan County,Shanxi Province were selected. All cases met the diagnostic criteria described in the"Consensus Concerns for Chronic Superficial TCM Diagnosis and Treatment(2009,Shenzhen)".The criteria included 58 patients randomized to treatment and control groups,29 in each group.Treatment group:Acupuncture was used to acupuncture at Zhiyang acupoint,Zhongwan acupoint,Weishu acupoints(double),Pishu acupoints(double),Zusanli acupoints(double),once every 3 days,9 days 1 course of treatment; After the second fire needle treatment, the acupoints were buryed for1 time(acupoints with the fire needle),after 20 days, the course of treatment was completed and observed for a total of 30 days.Control group:oral western medicine method, omeprazole 20mg/times;amoxicillin 0.5g/times,half an hour before meals and bedtime.4 weeks as a course of treatment,a total of 1 course of treatment.Observed the clinical efficacy of the two groups. The clinical efficacy of the two groups was observed. After the end of treatment, the clinical efficacy of the two groups of patients was compared and observed, including clinical symptomatic efficacy evaluation, quality of life score, and C14breath test evaluation. One month later, the recurrence of effective patients was followed up using a questionnaire and the recurrence rate was counted.Results:(1)The total effective rate was 92.86%in the treatment group and92.86%in the control group. There was no significant difference between the two groups(p>0.05),suggesting no significant difference between the treatment group and the control group;(2)The scores of clinical symptoms of the treatment group and the control group improved after treatment(P<0.01).There was no significant difference in the improvement of clinical symptom scores between the treatment group and the control group(P>0.05).However,the treatment group had better symptoms of both abdominal pain and poor appetite than the control group. The difference was statistically significant(P<0.01).The control group had excellent improvement in both symptoms of belching and acid reflux. In the treatment group, the difference was statistically significant(P<0.01),and there was no significant difference in other scores;(3)The total scores of the SF-36 scale before and after treatment in the two groups were significantly different(P<0.05),indicating that the quality of life of the patients after treatment in both the treatment group and the control group improved. Before and after treatment in the treatment group, the scores of the 8 subscales on the SF-36 scale were significantly improved compared with those before treatment, and the difference was statistically significant(P<0.05).In the SF-36 scale before and after treatment in the control group, the other seven subscales except the BP subscale were significantly improved compared with those before treatment, and the difference was statistically significant(P<0.05).However,the score of somatic body pain(BP)was not significantly improved, and the difference was not statistically significant(P>0.05).(4)The results of the C144 breath test before and after treatment in the treatment group and the control group showed that the Hp clearance rate was 71.43%in the treatment group and 92.86%in the control group;the H.pylori eradication effect was significant in both groups after treatment, and the difference was statistically significant(P<0.05);However,the scavenging effect of Hp in the control group was significantly better than the treatment group, and the difference was statistically significant(P*<0.05);(5)During follow-up,24patients were successfully followed in the treatment group, and 25 patients were successfully followed up in the control group. In the treatment group, there were 2cases of contact due to replacement contact, and 1 case of successful follow-up recurred. In the control case, there were 1 case of out-of-travel travel loss and 4cases of successful follow-up recurred. The recurrence rate in the treatment group was2.17%,which was significantly lower than that in the western medicine control group. The difference was statistically significant(P<0.05).Conclusion:(1)Fire needle combined with acupoint catgut embedding method has little difference in efficacy between CSG and oral western medicine. It can greatly improve the clinical symptoms of patients and relieve the suffering of patients;(2)Fire needle embedding therapy is superior to oral western medicine in improving the symptoms of epigastric pain and poor appetite;(3)For the improvement of the quality of life, the improvement of the physical pain is better than the oral western medicine method;(4)For the clearance rate of Hp,oral western medicine method is obviously superior to fire needle combined with acupuncture point embedding method;(5)In terms of safety, the combination of fire needles and acupoints embedding method has the characteristics of strong safety and no side effects;(6)In the 1-month follow-up,we observed that the recurrence rate of CSG in patients with fire needles and catgut embedding method was significantly lower than oral western medicine method..In summary, fire needle combined with acupuncture point embedding method has good analgesia and improve the symptoms of indigestion. At the same time,it has the characteristics of strong safety and no side effects, and it is worthy of promotion in clinical practice. |
