ObjectiveTo describe the status of the rehabilitation exercise compliance among lung transplant recipients after discharge.Based on the health belief model,this study explored the influencing factors of rehabilitation exercise compliance of lung transplant recipients after discharge.The purpose of this study is to provide the theoretical basis of discharging guidance and rehabilitation program for lung transplant recipients after discharge.MethodBy convenient sampling,this study selected lung transplant recipients who were followed up in outpatient department from January to September 2018.Questionnaires were conducted among eligible patients.When lung transplant recipients were reexamined in the clinic,they were surveyed by questionnaires,including general information questionnaire,rehabilitation exercise compliance questionnaire for lung transplant recipients after discharge,the Champion Health Belief Model Scale and the Perceived Social Support Scale.EpiData 3.1was used to establish the database,and SPSS22.0 was used for data statistical analysis.In this study,mean±standard deviation((?)±S) and composition ratio(%) was used to describe descriptive statistics.The independent samples T-test and one-way analysis were used to explore whether general data had obvious compliance with rehabilitation exercise for different patients.Correlation analysis were used to analyze the relationship among health beliefs,social support and compliance of lung transplant recipients.Multiple linear regression analysis clarify the influencing factors of rehabilitation exercise compliance of lung transplant recipients after discharge.P<0.05 for the difference was statistically significant.Result1.After discharge,the average total score of rehabilitation exercise compliance of lung transplant recipients after discharge was(35.13±8.30) points.Only 13.6% lung transplant recipients’total scored reached from 44 to 60 points.63.9% of lung transplant recipients whose total scored mainly were from 30 to 44 points.Among them,the scores of compliance in rehabilitation exercise,compliance in rehabilitation monitoring and compliance in seeking advice were(19.59±4.69) points,(6.85±2.26) points and(8.69±2.28)points,respectively.2.The results of independent samples t-test and one-way analysis showed that there were a significant differences in the scores of rehabilitation exercise compliance among lung transplant recipients of different ages,caregivers,surgical methods and postoperative time(P<0.05).Among them,the total score of compliance of lung transplantation recipients with caregivers was significantly higher than that of non-caregivers(t=2.058,P<0.05).The compliance of recipients after bilateral lung transplantation was significantly higher than that of recipients after single lung transplantation(t=-3.354,P<0.05).As for ages,the total score of rehabilitation exercise compliance for lung transplant recipients aged and over 60 years old was(39.96±7.31) point;(35.69±8.99) points for those aged 40 to 59 years;and(31.21±7.80) points for those aged 39 years and under.The least significant difference method used to compare the differences among different age groups,the differences in different age groups were statistically significant(P<0.05).As for postoperative time,the total score of compliance within 1-2 years after lung transplantation was(37.05±7.58) points,followed by within 1 year after operation.With the prolongation of operation time,the compliance was gradually weakened.The patients whose postoperative time over 3 years was only(28.89±8.28) points,which was the worst.By the least significant difference method,there was significant difference between groups with operation time more than 3 years and other three groups(P<0.05).3.Relevance analysis illustrated that the total score of health belief,perceived susceptibility,perceived severity,perceived benefits,health motivation,self-efficacy were positively correlated with the total score of rehabilitation compliance(r=0.523,0.171,0.491,0.485,0.172,0.380,P<0.05);total score of social support,in-family support,extra-family support,and the difference was statistically significant were positively correlated with the total score of rehabilitation compliance(r=0.309,0.455,0.162,P<0.05).4.By stepwise regression analysis whit the multiple linear regression model,further explore the influencing factors of rehabilitation exercise compliance for lung transplant recipients after discharge.Total of 7 variables entered the regressive equation(R~2=0.512),which meant main factors influencing the compliance.The results demonstrated that the effects of independent variables on rehabilitation exercise compliance ranged from big to small in order:self-efficacy,in-family support,surgical methods,perceived severity,perceived benefits,age and postoperative time.Conclusion1.The rehabilitation exercise compliance of lung transplant recipients after discharge is at the middle and low level.2.The main factors affecting the rehabilitation exercise compliance for lung transplant recipients to outside hospital include self-efficacy,in-family support,surgical methods,perceived severity,perceived benefits,age and postoperative time.The patients ages been older,the postoperative time been shorter,the perceived severity been greater,and the perceived benefits,the self-efficacy and the in-family support been higher,the compliance with rehabilitation exercise is higher.
