Objective:Through the effect of Jiawei Shenqi Dihuang Decoction on the expression of AGEs and PKC in urine of DKD patients in early and middle stages,the possible mechanism of Jiawei Shenqi Dihuang Decoction in reducing urinary protein and delaying renal function in DKD patients was explored,so as to provide effective TCM treatment for DKD patients in early and middle stages.Method:From March to October 2018,61 patients with DKD in the second Department of Nephrology of Heilongjiang Academy of Traditional Chinese Medicine(Heilongjiang Academy of Traditional Chinese Medicine)were randomly divided into Jiawei Shenqi Dihuang Decoction treatment group(31 cases)and Keluoxin control group(30 cases).Both groups were given basic treatment.The treatment group was given basic treatment combined with Jiawei Shenqi Dihuang Decoction,while the control group was given Keluoxin for 3 months.Before and after treatment,the clinical efficacy,TCM symptom score,urine protein,blood sugar,blood lipid,renal function,liver function,AGEs,PKC and safety indexes of the two groups were analyzed and evaluated statistically.Result:1.The total effective rate of the treatment group was 83.87%,and that of the control group was 72%.The difference was statistically significant(P<0.05).2.The total scores of TCM syndromes in the two groups after treatment were significantly lower than those before treatment(P<0.05).There were significant differences between the treatment group and the control group(P<0.05).There were significant differences in improving fatigue and limb numbness(P<0.01).There were significant differences in improving waist and knee weakness(P<0.01).3.After treatment,UTP,MAU and creatinine in the two groups decreased significantly compared with those before treatment(P<0.05),and there was significant difference between the MAU treatment group and the control group(P<0.05).There was no significant difference in 24-hour UTP and creatinine betweenthe treatment group and the control group(P>0.05);there was no significant difference in urea nitrogen between the two groups after treatment compared with before treatment(P>0.05).4.After treatment,the levels of FPG,TC and TG in the two groups decreased significantly compared with those before treatment(P<0.05).There was significant difference between the treatment group and the control group(P<0.05).There was no significant difference in the levels of TC and TG between the treatment group and the control group(P>0.05).There was no difference in the levels of HbAlc between the two groups after treatment compared with that before treatment.The significance of accounting(P>0.05).5.After treatment,the levels of AGEs and PKC in the two groups decreased compared with those before treatment,and the difference was statistically significant(P<0.05).The difference between the two groups was statistically significant(P<0.05);the difference between the AGEs groups was statistically significant(P<0.01).6.There was no significant difference in liver function between the two groups after treatment and before treatment(P>0.05).ECG,blood routine,stool routine and occult blood were all within normal range during treatment,and no adverse events occurred.Conclusion:1.Jiawei Shenqi Dihuang Decoction can reduce proteinuria,improve renal function and regulate glycolipid metabolism.2.Jiawei Shenqi Dihuang Decoction can improve the clinical symptoms of DKD patients with deficiency of both Qi and Yin and blood stasis in the early and middle stages.3.Jiawei Shenqi Dihuang Decoction can down-regulate the expression of AGEs and PKC in urine.
