| After the reform and opening up,with the continuous development of China’s economy,socialism with Chinese characteristics has entered a new era.The main contradiction in our society has been transformed from the contradiction between the people’s growing material and cultural needs and backward productivity to the contradiction between the people’s growing needs for a better life and the unbalanced and insufficient development.Similarly,in the field of medical and health care,there is also a huge imbalance and insufficiency in the allocation of health resources.This is a great challenge to build a harmonious society in China.Therefore,the research on how to improve the efficiency of health resources utilization is particularly important.These issues have also attracted the attention of various circles of society and scholars,and are also a key area of their research.Through reading a large number of documents,it is found that scholars at home and abroad are always interested in this issue and have achieved fruitful results in this field.Unfortunately,they have not reached a consistent conclusion.Some scholars believe that the low efficiency of medical and health resources utilization in China is due to the small scale of investment.However,these scholars have neglected the factors that may affect the efficiency of health resources utilization,such as interregional social background,economic situation,health system status,etc.Jiangxi Province’s social and economic development is at the middle level of China’s provinces as a whole,and there are also great differences in social background,economic development and health system among cities in the province.Therefore,Jiangxi Province is chosen as the research object in this paper,which is representative and can make the research more academic and valuable.In view of this,on the basis of the existing research,this article obtains the relevant data of the article research by consulting "Jiangxi Province Statistical Yearbook","Jiangxi Province Health and Family Planning Statistics" and the municipal statistical offices.Firstly,Lorenz curve and Gini index are used to measure the equilibrium of medical and health resources allocation in Jiangxi province.Then,the Two-stage DEA method is used to measure the static efficiency of health resources utilization in Jiangxi province from 2010 to 2016.Then the dynamic efficiency of medical and health resources utilization in Jiangxi Province is decomposed by using redundancy value and Malmquist index to obtain the influence of various factors such as external environment,random error,internal factor management on the specific quantity of input value and output value,and what factors promote the improvement of medical and health resources utilization efficiency.Finally,on the basis of considering the spatial correlation,this paper introduces the spatial econometric model that can explain the spatial correlation theory,and decomposes the explanatory variables through the spatial Dubin model to obtain the direct effect,indirect effect and total effect on the utilization efficiency of medical and health resources.We have come to the following preliminary conclusions:(1)In terms of scale efficiency,comprehensive efficiency and pure technical efficiency,the overall utilization efficiency of health resources in Jiangxi Province in recent years is lower than the optimal level of 1.0.(2)In 2016,only Yichun City and Shangrao City in Jiangxi Province have the best utilization efficiency of health resources,while the other nine cities have the situation of insufficient utilization efficiency of health resources in varying degrees.(3)From the data of these years,we can know that the utilization efficiency of health resources in the cities of Jiangxi Province is increasing,and the utilization efficiency of health resources in the cities adjacent to Nanchang is generally higher than that in the cities not adjacent to Nanchang.(4)Through the analysis of redundancy value,it is found that the input redundancy value of health resources in Jiangxi Province is accompanied by insufficient output.(5)Through the decomposition of Malmquist index,we can see that the total factor productivity of medical and health resources in Jiangxi Province has been increasing for four years in the past six years,and has contributed to technological progress for three years.(6)Through Geary index C test,we know that there is a spatial correlation between the utilization efficiency of health resources in Jiangxi Province.(7)The direct effect,indirect effect and total effect of explanatory variables are decomposed by using spatial Durbin model.It is found that in the direct effect,the total number of outpatient clinics and per capita GDP have a negative impact on the utilization efficiency of health resources in Jiangxi Province.Urbanization level,aging level,population density and density of medical institutions have a negative impact on the utilization efficiency of health resources in Jiangxi Province.In indirect effect,per capita GDP,aging level and population density have a negative impact(inhibition)on the utilization efficiency of health resources in Jiangxi Province.The total number ofoutpatients,urbanization level and density of medical institutions have an indirect positive impact(promotion)on the utilization efficiency of health resources in Jiangxi Province.In total effect,per capita GDP has an indirect positive impact on the utilization efficiency of health resources in Jiangxi Province.Aging level and population density have a negative impact on the utilization efficiency of health resources in Jiangxi Province.The total number of outpatient clinics,urbanization level and the density of medical institutions have indirect positive impact on the utilization efficiency of health resources in Jiangxi Province.Based on the empirical analysis and the relevant references,this paper puts forward the corresponding policy recommendations,in order to provide a reference for the efficient use of government health resources. |
