ObjectiveWhen the residual alveolar bone height in the maxillary posterior region is 2-5mm,lateral window technique is often used to solve this problem in clinical practice.But this method has defects such as big trauma,long operation time and strong discomfort.The above defects can be avoided by using " simultaneous osteotome sinus floor elevation with PRF and bio-oss bone powder material" to solve this problem.The purpose of this study was to investigate the clinical effect of " simultaneous osteotome sinus floor elevation with PRF and bio-oss bone powder material " when the residual alveolar bone height in the maxillary posterior region was less than 5mm.MethodsWe selected twenty-three patients whose teeth residual alveolar bone height is less than 5 mm in the maxillary posterior area as the research object from august 2011 to august 2016 in The First Affiliated Hospital of Zhengzhou University Oral Implantology Department,and all the patients did simultaneous osteotome sinus floor elevation with PRF and bio-oss bone powder material.We collected patients X-ray imaging datas and indexs of soft tissue around implants,Implant Stability Quotient and evaluated the following indicators: The mean height of osteotome sinus floor elevation;The postoperative pain,Cumulative Survival Rate,Implant Stability Quotient,Marginal Bone Loss,soft tissue index(modified plaque index,modified gingival index,modified sulcus bleeding index).SPSS19.0 statistical software was used for statistical analysis of the data.ResultsThe mean residual alveolar bone height of the 23 patients was 4.03±1.06 mm,with a minimum of 2.76 mm and a maximum of 5.00 mm.The mean height of osteotome sinus floor elevation was 4.98± 1.37 mm,with a minimum of 4.16 mm,and a maximum of 6.24 mm.The Postoperative pain degree of the patients: 12 were first-grade,9 were second-grade and 2 were third-grade.The implant survival rate was 100%.The implant stability quotient was 73.89±2.02 six to eight months(mean 7.28±0.53)after implantation.One year after wearing dental crown,the bone resorption at the neck margin of the implant was 0.91±0.33 mm;the modified plaque index was 1.17±0.63,the modified gingival index was 0.39±0.64,and the modified gingival sulcus bleeding index was 1.09±0.71.ConclusionThe technique of “simultaneous osteotome sinus floor elevation with PRF and bio-oss bone powder material” is a safe and effective method in solving the severe vertical bone deficiency(2-5mm)in maxillary posterior teeth in the case of strict selection of indications.It reduces the risk of complications such as perforation and rupture of maxillary sinus mucosa and postoperative infection.The patients had low postoperative pain,and the Cumulative Survival Rate,Implant Stability Quotient,Marginal Bone Loss,soft tissue indexs were all within the normal range.The short-term clinical effect was good in the clinical observation,while the long-term effects remain to be further observed.
