Objective:To compare the application value of neoadjuvant chemotherapy regimen(TP,TC)in locally advanced cervical cancer,and to provide the basis for the determination of clinical treatment regimen for this kind of patients.Methods :68 patients with locally advanced cervical cancer admitted to Huaihe Hospital from January 2014 to July 2018 were treated with radical surgery and two courses of neoadjuvant chemotherapy before operation.According to the chemotherapy regimen,they were divided into two groups: control group(n = 32)treated with TP regimen chemotherapy(paclitaxel cisplatin);36 patients in the observation group were treated with TC regimen chemotherapy(paclitaxel carboplatin).The operative indexes(operation time,intraoperative bleeding volume),total effective rate,toxicity and side effects,and postoperative pathology were counted in the two groups,and the patients in the observation group were treated with paclitaxel carboplatin(paclitaxel).1-year and 2-year survival rates.Results:(1).There was no significant difference in the operation time and intraoperative blood loss between the observation group and the control group(P>0.05).(2).The tumor diameters of the two groups were significantly reduced after neoadjuvant chemotherapy,and the tumor diameter of the observation group was not significantly different from that of the control group after neoadjuvant chemotherapy(P>0.05).The operative time and the amount of bleeding in the observation group were not significantly different from those in the control group(P > 0.05).The total effective rate and clinical control rate in the control group were 84.38% and 90.63% respectively.There was no significant difference in the total effective rate and the clinical control rate between the two groups(P > 0.05).(3).There were no significant differences in the number of patients with positive cervical margin,positive vaginal margin,deep invagination of the cervix,uterine infiltration,vascular involvement and lymph node metastasis in the control group and the observation group(P>0.05).(4)There were no significant differences in hemoglobin reduction,thrombocytopenia,leukopenia,impaired renal function,hair loss and degree between the two groups(P>0.05).The gastrointestinal side effects,peripheral neurotoxicity,muscle and joint pain and other side effects were observed in the observation group.The incidence was significantly lighter than the control group(P<0.05).(5).The 1-year survival rate and 2-year survival rate were significantly lower in the two groups.There was no significant difference in the 2-year metastasis rate between the two groups(P > 0.05).Conclusion:In the treatment of locally advanced cervical cancer patients,the efficacy of TP chemotherapy regimen is similar to that of TC regimen in preoperative neoadjuvant chemotherapy.There is no significant difference in operation,postoperative pathology and survival between the two regimens,and there is no significant difference between the two regimens in the treatment of locally advanced cervical cancer.However,the toxicity and side effects of TC regimen were relatively mild,and the patients were more susceptible to tolerance.In addition,there is no need to implement a large number of spas during the application of the TC program,the operation is simpler,the patient acceptance is higher,and it is easier to promote.
