Clinical Observation Of Method Of Bone Extractions In The Treatment Of Bone Exposed Wounds With ASO

Objective : To observe the clinical effect of Method of bone extractions in the treatment of bone exposed wound in patients with ASO,and to establish its operating standards,and to promote the use of this method in clinical.Methods:60 patients of lower extremity Arteriosclerosis Obliterans with stage III and grade I with bone exposed in were selected as the research objects.Randomized controlled trial was used.All patients received routine medical treatment:through oral medicine control of blood pressure,lipid-lowering,vasodilation,oral Chinese medicine Gubu Decoction combined with dialectical addition and subtraction.In Surgical treatment: 33 cases were treated with bone extraction and healing method to remove exposed bone and Shengji Yuhong ointment gauze strip for external dressing change.Control group: 27 cases were treated with Shengji Yuhong ointment gauze strip for external dressing change.By observing the wound healing,limb pain,skin color,improvement of intermittent claudication,ankle-brachial index and other indicators,the difference of curative effect between the two groups was compared by statistical method,and the wound healing of patients with lower limb arteriosclerosis obliterans stage III and grade I bone exposed was objectively evaluated by bone extraction and healing method.Results:UnderAfter two courses of treatment,there were no cases of exfoliation or removal.The skin color,pain,secretion,depth and granulation growth of the wound in the treatment group were improved compared with those in the control group(P < 0.05).The improvement mainly came from the changes of skin color,wound depth and area.There was no significant difference between the two groups in the improvement of ABI and intermittent claudication(P > 0.05).But in wound healing rate of the treatment group was36.4%,the total effective rate was 93.9%;the control group was 11.1%,the total effective rate was 85.2%;the cure rate of the two groups was statistically significant(P < 0.05),the healing time of ulcer in the treatment group was significantly less than that in the control group(P < 0.05),the total effective rate of the two groups was not significant(P > 0.05),the treatment group was better than the control group as a whole.Conclusion : The effect of bone extraction and healing method on bone exposedwounds in patients with arteriosclerosis obliterans is clear.It can accelerate healing time and improve wound healing rate even though the symptoms of two groups of ABI and lower limbs are not improved obviously.Therefore,the effect of bone extraction and healing method on Surgical treatment of wounds is definite,and it is worth popularizing in clinical use.
