Research On The Medical Insurance System Operation In Yunnan Province—Based On The Rational Choice Of Institutionalism

In the implementation of the medical security system,there must be a contradiction between the pursuit of economic rationality and the pursuit of the maximization of social value.From the perspective of rational choice institutionalism,the realization of the balance between them and the contradiction of the negative impact can only be achieved through institutional rationality.Institutional rationality can suppress problems such as "reverse selection" or "moral hazard".The exertion of institutional rationality can only be achieved through the construction and improvement of the system.The core element of these efforts is the strengthening of regulation.The strengthening of supervision includes not only the formulation of laws and regulations,the construction of new regulatory structures,the design and diversification of regulatory means,but also includes the problem of resolving the information asymmetry of each participant.As policy makers and policy implementers,government departments must assume a leading and central role in institution building and improvement.Through institutional construction and improvement,the preferences as "rational people",can be guaranteed and the public policy goals can be realized.Since the first implementation of basic medical insurance for urban workers in Yunnan Province in 1999,the population of the medical insurance system has been fully covered and made significant contributions to the basic health and medical care of citizens through continuous efforts,although this guarantee is still low level.Those efforts have maintained social stability and promoted economic development.In addition to the constraints of limited resources,other institutional deficiencies still hinder the full play of the effectiveness of the medical security system.For general and specific issues in Yunnan Province,Only by adopting multi-faceted and multi-faceted measures with the core objectives of institution building and improvement can institutional rationality be fully utilized.The full play of institutional rationality is to meet the reasonable interests of all participants in medical insurance,ensure the continuous and stable operation of the medical security system under local jurisdiction,improve the level of medical insurance and achieve a relative maximization of social value rationality.
