Objective Use quality improvement model to train the outpatient doctors of Endocrinology Department in basic insulin treatment pathway and health education methods,and to consolidate the doctors’ theoretical knowledge and improve their professional skills.Then to observe the effect of these quality improvement measures for outpatient doctors on glycemic management in type 2 diabetic patients.Methods The first 100 patients with type 2 diabetes mellitus who used basic insulin from the Endocrinology Clinic of the Second Affiliated Hospital of Anhui Medical University from January to March 2017 were selected.The demographic information of age,height,weight and gender,fasting blood glucose,glycosylated hemoglobin,blood lipid,blood pressure,hypoglycemia,EQ-5D health scale score,hypoglycemic regimen and the confidence of outpatient doctors in basic insulinwere collected.The outpatient doctors are trained in basic insulin-related knowledge,and the director of the Department regularly checks and holds exchange meetings to feedback the difficulties encountered by the doctors in the implementation of outpatient diagnostic and therapeutic measures,to analyze the possible reasons that hinder the implementation of hypoglycemic goals,and to find solutions.Thus,after a six-month quality improvement cycle,another 100 patients with type 2 diabetes mellitus treated with basic insulin from October to December 2017 will be engaged again.The differences of the above-mentioned observation indexes after and before intervention were compared.Results After the intervention of quality improvement measures,the doctor’s professional skill score was higher than before.The average fasting blood glucose,glycated hemoglobin,total cholesterol,triglyceride,low density lipoprotein andsystolic blood pressure of the patients were lower than before.The compliance rate of fasting blood glucose and glycated hemoglobin,and the satisfaction of patients with self-health status were significantly higher than those before intervention(P < 0.05).However,there was no significant difference in diastolic blood pressure,body mass index,hypoglycemia rate and doctors’ confidence in basic insulin before and after intervention(P > 0.05).Conclusion The quality improvement measures for outpatient doctors can improve the professional skills of outpatient doctors and further improve the overall quality of glycemic management in patients with type 2 diabetes mellitus.
