BACKGROUND AND OBJECTIVESAcne is one of the most common skin diseases among teenagers and young adults.The hair follicle epithelial excessive diversification,sebaceous glands secretion,acne propionic acid bacillus engraftment and relevant activities and inflammatory response are main courses.Rosacea is usually sawn in central face,mainly involving the facial blood vessels and pilosebaceous unit of chronic inflammatory diseases,which is related to immune disfunction,abnormal vascular-nerve regulation,microbial infection,skin barrier damage and genetic factors are most welcome.The main influencing factors are sunlight,emotion,diet,alcohol and tobacco,environment temperature,family history,etc.Both acne and rosacea have a long disease course,which are prone to relapse,and can develop into a derogatory skin disease,which has a significant impact on patients’ mental health and quality of life,especially in female patients.Acne vulgaris and rosacea are both multifactorial diseases and neither fully understood.Clinically,it is not uncommon to find that rosacea and acne overlap.However,there is limited information about acne with rosacea at home and abroad.Through the outpatient investigation,this study aims to understand the classification and severity of rosacea in women with acne vulgaris,study the factors affecting the classification and severity of rosacea,and provide targeted nursing and health education guidance for female rosacea patients with acne vulgarns.METHODSA study was conducted on female patients who were admitted to the outpatient department of dermatology of our hospital from December 2016 to July 2018 and were clinically diagnosed with acne combined with rosacea.Demographic data,clinical characteristics,living habits,dietary habits and family history of the patients were collected through a self-designed questionnaire.Through cross-sectional investigation,the severity and classification of rosacea were investigated.Single factor analysis of variance(anova)was used to study the factors influencing the classification and severnty of acne with rosacea.Then Multi-factor logistics regression analysis was conducted for these related factors to obtain the main influencing factors for the classification and severity of rosacea associated with acne.RESULTSA total of 563 cases of rosacea in women with acne vulgaris were included in this study.1.Proportion of rosacea severity in acne patients:408 cases(73.1%)of erythema capillary dilatation type,128 cases(22.9%)of pimple type,and 22 cases(3.9%)of hyperplasia type.Single factor disorderly Logistic regression analysis results showed that makeup frequency,rosacea family history,the use of skin care products or cosmetics and the age of onset of acne were the influencing factors of the classification of acne with rosacea(P<0.1).which were included for further multi-factor Logistic regression.Log-likelihood ratio test results showed:rosacea family history,the use of skin care products or cosmetics were factors influencing the classification of rosacea.Compared with phymatous rosacea,patients with a family history were less likely to develop erythematotelangiectatic rosacea;patients using skin cosmetics or care products were more likely to suffer from erythematotelangiectatic rosacea and papulopustular rosacea than phymatous rosacea.2.Among the patients with acne accompanied with rosacea,116(20.6%)had mild rosacea,279(49.6%)had moderate rosacea,and 168(29.8%)had severe rosacea.Univariate Logistic regression analysis showed that ages of aggravation of acne,spicy food intake,rosacea family history,makeup frequency were the influencing factors of the classification of acne with rosacea(P<0.1).which were included for further multi-factor Logistic regression.The results showed The severity of rosacea was higher in people fond of spicy food than those who do not;Patients with a rosacea family history had lower rates of rosacea than those without a family history.The higher the frequency of sun exposure,the higher the severity of rosacea.CONCLUSIONS1.Factors affecting the classification of rosacea in female acne patients include rosacea family history and use of skin cosmetics or care products.Factors influencing the severity of rosacea include sun exposure frequency,spicy food intake and a family history of rosacea.2.For patients with rosacea combined with acne,attention should be paid to the health education of use of skin cosmetics or care products,sun exposure and diet.Acne patients with no family history of rosacea should also be actively controlled by avoiding potential risk factors. |
