Estimation Analysis On Curative Care Expenditure Of Tumor In Jilin Province Based On The System Of Health Accounts 2011

With the development of social economy,the advancement of industrialization and urbanization,the acceleration of the aging process of the population,the spectrum of human diseases and the spectrum of death have also undergone tremendous changes.The chronic non-infectious diseases represented by tumors have become the main threat to human health.its morbidity and mortality are on the rise in both developed and developing countries.China as the biggest developing country in the world,in recent years,the incidence and mortality of tumors are on the rise.The patient brings great pain and also brings a great disease burden to the patient’s family and society.Objective:This study aims to from the perspective of tumor diseases expenditure,based on 2017 Jilin Province tumor curative care situation were analyzed,and the cost of the service function classification,to analyze the flow distribution,beneficiaries,etc,and the evaluation of tumor curative care either from the macroscopic angle to understand the amount of Jilin Province tumor curative care scale,and the internal structure of a cost from the angle of the micro a quantitative and comparative analysis.Providing data support for tumor health care policy and tumor prevention and control plant through the result of this study.Methods:Based on extensive reading of domestic and foreign references,this study used multi-stage stratified sampling to select outpatient and hospitalization cost data for various types of medical institutions at all levels in Jilin Province.Based on the theory of “A System of Health Accounts 2011”.The framework uses a “top-down” decomposition accounting method to analyze the cost of tumor curative care in Jilin Province in 2017.Data collection and collation use EXCEL2016,statistical analysis using Stata12.0 analysis software,with the service cost of the service function,institutional distribution,beneficiary population,etc.,to fully understand the incidence of tumor curative care expenditure.The factors affecting the average cost of tumor curative care were analyzed by Grey Relational Analysis.Results:1.In 2017,the cost of tumor curative care in Jilin Province was 678.828 million yuan,the average curative care cost was 10514.46 yuan;the average outpatient curative care cost was 848.93 yuan;the average hospitalization expense was 20184.33 yuan.2.In terms of gender and age distribution,the proportion of male in tumor curative care expenditure is 44.34%;the proportion of female is 55.66%.The average cost of curative care for men is 14121.00 yuan,and the average curative care cost for women is 8693.61 yuan.From the age distribution,the 0-14 age group accounted for 1.44% of the total population curative care expenditure;the 15-59 age group accounted for 55.66% of the total population curative care expenditure;the age group 60 and above accounted for 42.90% of the total population curative care expenditure.The average curative care cost for the 0-14 age group population was 3980.45 yuan;the average curative care cost for the 15-59 age group population was 9733.46 yuan;the average curative care cost for the age group 60 and above was 12147.43 yuan.3.From the perspective of disease distribution,malignant tumors accounted for 82.70% of the total cost of tumor curative care;benign tumors accounted for 12.17% of total tumor curative care expenditure;in situ tumors accounted for 1.51% of total tumor curative care expenditure;dynamic undetermined or dynamically unknown tumors accounted for tumor curative care the total cost is 3.62%.Among them,the top five diseases according to the disease chapter are: malignant tumors of the digestive organs,malignant tumors of the respiratory and intrathoracic organs,malignant tumors of the breast,malignant tumors of the female reproductive organs,malignant tumors of lymph nodes,hematopoiesis and related tissues.4.From the perspective of institutional distribution,provincial-level medical and health institutions account for 55.10% of tumor curative care expenditure.Municipal medical and health institutions accounted for 12.56% of the cost of tumor curative care.District and county-level medical and health institutions accounted for 27.73% of the cost of tumor curative care.Primary medical and health institutions accounted for 4.61% of the cost of tumor curative care.General hospitals accounted for 74.99% of the cost of tumor curative care.Specialized hospitals accounted for 14.88% of the cost of tumor curative care.General hospitals and specialized hospitals account for 89.87% of the curative care expenses of all types of medical and health institutions,and are the main medical and health institutions where tumor expenses occur.5.According to Grey Relational Analysis method,the factors influencing the average hospitalization cost per time of tumor patients in Jilin province in 2017 are in descending order: drug cost,curative care and care cost,examination and laboratory cost,other expenditure,and operation cost.Conclusions:1.The tumor of curative care expenditure in Jilin Province is large,the average curative care expenditure and the average hospitalization expenditure are high,and the consumption of disease resources is large.2.Patients of different genders have differences in the expenditure of tumor curative care,specifically: the cost of female curative care is higher than that of male;The average expenditure per curative care for men was higher than that for women.3.There are differences in the expenditure of tumor curative care among different age groups.Specifically,the curative care cost of the 15-59 age group is higher than that of other age groups,and the average curative care expenditure of the age group 60 and above is higher than other age groups.4.The differences on different types of tumor curative care expenditure,malignant tumor curative care scale is higher than other types of tumor,among them,according to the disease chapter to differentiate malignant tumor curative care expenditure in the top five of the disease is in turn: malignant tumor,respiratory and digestive organs chest cavity malignant tumor,breast malignant tumor,the female reproductive organs organ malignant tumor,lymphatic and hematopoietic malignancies and relevant organizations.5.There are differences in the expenditure of tumor curative care between different levels and types of institutions,specifically: the higher the level of medical and health institutions,the greater the cost scale;Outpatient expenses of primary medical and health institutions are relatively low;The cost of tumor curative care is more in hospitals,especially general hospitals,and the cost of curative care in other types of medical institutions is smaller.6.The factors affecting the expenditure of hospitalization per tumor time are: drug cost(including western medicine,Chinese patent medicine,Chinese herbal medicine),curative care and care cost,examination and laboratory cost,other expenditure(including bed cost,registration cost,material cost,oxygen cost,blood transfusion cost,etc.),operation cost.
