Health is an indispensable requirement for the promotion of man's all-round development and a basic condition for economy and social development.Children's health is the guarantee of the future society and economic development.However,with decreasing of maternal women and child mortality rates,the incidence of birth defects has risen and it has increasingly become a prominent public health issue.It's an important prerequisite for preventing and promoting the health of the birth population and the basis for effectively reducing the burden of patients with birth defects on the cost of treatment if the current status of birth defects treatment costs is understood by people correctly.Objective:Exploring the cost of treatment for birth defects and understanding current birth defects prevention policies in China.Reveal and master the current status of birth defects treatment costs in Jilin Province from macroscopic to microscopic by this study.We analyse the cost composition differences of different types of birth defects diseases.Provide data support for the formulation of birth defects prevention policies and the adjustment of protection policies through the result of this study.Methods:We choosed different types of medical and health institutions which provide the curative care expenditure of 54933 birth defects patients by a multi-stage stratified sampling method.The indicators of this survey data include: age,gender,length of staying in hospital,disease name,ICD-10 code,outpatient expenses,inpatient expenses,treatment cost,medicine cost(including western medicine,proprietary Chinese medicine,Chinese herbal medicine fee),registration cost,inspection cost,surgical cost and so on.This study is based on SHA2011(A System of Health Accounts 2011 Edition).Stata12.0 is used to operate and analyze patient information of sample institutions.Result:The cost of birth defects in 2014 in Jilin province reached 600 million yuan and the average cost was 10914.05 yuan.From the perspective of service distribution,the proportion of the cost of birth defects treatment which spent on hospitalization is reached 77.53%.From the perspective of 5-year-old age distribution,the proportion of the cost of the treatment for children aged 0-4 years is 12.5%.From the perspective of disease distribution,the proportion of the cost of congenital malformation,deformation and chromosome abnormalities accounted for 30.52%.The cost of hemolytic anemia and some diseases involving the immune mechanism accounted for 46.05%.From the perspective of the cost structure,37.04% is for medicines,27.57% for materials and other expenses,17.92% for inspection and testing,and 17.47% for surgery.From the angle of fund-raising,the proportion of out of the pocket reached 41.87%.From the perspective of institutional distribution,the cost of treatment in provincial-level medical and health institutions accounted for 62.07%,the cost of treatment in municipal medical and health institutions accounted for 21.57%,the cost of treatment in district-level medical and health institutions accounted for 15.11%,the cost of treatment at the primary health care institutions accounted for 1.24%.From the perspective of gender distribution,men's treatment costs accounted for 47.18% and women's treatment expenses accounted for 52.82%.In addition,the expenditure show differences in the service distribution and service structure of expenses in different age groups,the different types of diseases and the costs of different levels of institutions.Conclusion:1.The disease protection system of China and Jilin Province are gradually improving.In recent years,the proportion of out of the pocket has gradually declined.However,the phenomenon of “poverty due to illness” will still occur in ordinary urban and rural families due to the relatively high absolute cost of the treatment of birth defects under the current level of protection.2.There are significant differences in distribution of curative care expenditure and the curative care expenditure in different age groups.In the specific prevention and control,while focusing on children,the government should further increase the concern and protection for the treatment of patients with birth defects in adults and the elderly.3.The Curative care expenditure for different diseases has significant differences in service distribution and cost structure.4.The primary health care institutions can be a more important role in the prevention,treatment and rehabilitation of birth defects.
