Low-dose Fluorescence-sodium Guided Resection Of High-grade Glioma:A Clinical Study

ObjectiveGlioma is a kind of malignant tumor with high multiplication speed and very poor prognosis.Surgical treatment is the main treatment method of glioma.This study was designed to evaluate the clinical utility of low-dose fluorescein-sodium guided high-grade gliomas.MethodA retrospective analysis of 67 patients with high-grade glioma diagnosed and treated by neurosurgery at the First Hospital of Jilin University from September 2012 to February 2019.All patients were enrolled in the pathological diagnosis of WHO III-IV glioma.They were divided into no-dose group,high-dose group and low-dose group.The general clinical characteristics such as age,gender,KPS score and pathological type were recorded and the patients were followed up.The no-dose group wasn’t given any sodium fluorescein,and the high-dose group was given a pre-dural injection of 20 mg/kg body weight of sodium fluorescein,and the low-dose group was injected with 2.5 mg/kg body weight of sodium fluorescein 120 minutes before the epidural membrane was opened.The surgical microscope used a Pentero 900 surgical microscope(ZEISS Meditec,Germany)containing a 560 nm filter.Intraoperative observation of tumor staining,MRI to determine the scope of tumor resection within 72 hours after surgery,postoperative KPS,complications and any side effects caused by sodium fluorescein.Result1.The complete resection rate of the non-fluorescence rate was 57.1%,the complete resection rate of the low-dose group was 87.5%,and the complete resection rate of the high-dose group was 86.4%.The complete resection rate of the low-dose group and the high-dose group were significantly higher than the complete resection rate of the non-fluorescent group.There was no significant difference between the complete resection rate of the low-dose group and the high-dose group.2.The postoperative KPS of the low-dose group was improved more significantly than the KPS of the high-dose group and the non-fluorescent group.3.The average operation time and average hospitalization time in the low-dose group were shorter than the high-dose group.4.One high-dose group’s patient side had fluorescein-sodium side effects,which was not observed in the low-dose group.Three Recurrent cases were observed in the non-fluorescent group at the 3rd month after operation,and one case was observed in the high-dose group at the 3rd month after operation,and None was observed in the low-dose group at the 3rd month after operation.Conclusion1.Compared with non-fluorescein-sodium-guided surgery,low-dose fluorescein sodium-guided surgery and high-dose fluorescein sodium-guided surgery can improve maximization of EOR;2.Compared with high-dose fluorescein sodium-guided surgery,Low-dose fluorescein sodium-guided surgery can shorten the patient’s operation time and hospital stay.3.Compared with high-dose fluorescein sodium-guided surgery,Low-dose fluorescein sodium-guided surgery can reduce the occurrence of sodium fluorescein-related adverse events.
