Background: The impact of stress response caused by special work and occupational environment on the body has always been concerned.As a national security guard force,the island-reef guards are a group with special missions and tasks.With the continuous development of China’s strength,the international situation in the coastal area has become more complicated.It is an inevitable trend to pay more attention to the mental and physical health of island-reef guards.Stress detection is the basis and premise of stress warning,intervention,protection and maintenance of physical and mental health.Traditional stress assessment methods are limited by factors such as long time-consuming,subjective and professional implementation.So,the reliability,timeliness and popularity of stress assessment need to be improved.Therefore,it is particularly important to study the response regularity of stress and to develop simple,rapid and objective stress assessment methods and/or equipment.Objective: Firstly,based on the multiple methods of stress level detection,we use the psychological questionnaires,behavioral instruments and saliva sample testing to assess the stress levels,as while combined with demographic characteristics such as enlistment time,education level,and marital status to analyze comprehensively the influence of island-reef environment on the physical and mental health of the guards.Secondly,from the aspects and levels of stress response,we analyze the correlation between psychological,behavioral and physiological indicators in order to find out objective and stable stress physiological indicators.Finally,using human saliva as a sample,we develop the non-invasive,real-time,fast and convenient self-test salivary test strip which based on the principle of immunochromatography technology,objective physiological indicators and three-dimensional microfluidic technology.The test strip would achieve the rapid detection of stress levels within 1-3 minutes and open up new ideas and methods for stress detection.Methods: 1.The psychological,behavioral and physiological indicators of stress were measured in 80 island-reef and 80 coastal land soldiers by random sampling method.Psychological stress indicators mainly include psychosocial stress survey for groups(PSSG),self-rating anxiety scale(SAS)and self-rating depression scale(SDS).Behavioral indicators include acoustic reaction time(RT)and hand coordination measurement.Physiological indicators salivary cortisol,IL-1beta and IL-6 concentration were measured by ELISA.Finally,the SPSS statistical software was used to compare and analyze the data.2.Statistical Pearson or Spearman method was used to analyze the correlation of psychological,behavioral and physiological indicators of stress.Then,combined with the literature to screen and identify the specific stress detection objective markers in saliva,which provided sufficient basis for the development of stress test strip.3.Using the human saliva as sample,we develop a rapid test strip for stress detection,which based on the three-dimensional microfluidic chip technology and immunochromatography principle.Then,apply the test strip to the military to verify its feasibility and accuracy which provides a basis to improve and perfect it the next step.Results:1.The scores of PSSG,SAS and SDS scale and the positive detection rate,the physiological indicators of salivary cortisol,IL-6,IL-1beta,as well as the average reaction time,operational error rate and the time of coordinated of hands in the island group were higher than those in the land group.The changes of stress indicators of island guards have demographic characteristics.The stress response is more obvious among soldiers with more than 2 years’ garrison time,higher education level and only-child in the family.2.Psychological,behavioral and physiological indicators are consistent for stress assessment.Salivary cortisol concentration has a high linear correlation with stress psychological and behavioral indicators,which can be used as an objective physiological indicator to evaluate stress level.3.Salivary immunochromatographic test strip with the salivary cortisol as detection target,the three-dimensional paper microfluidic technology as the basis and nano-carbon nanoparticles as label marker basically meets the requirement of rapid stress detection within 1-3 minutes,which achieves the goal of non-invasive,real-time,fast,convenient,self and objective stress detection.Conclusion:Island-reef guards have relatively higher stress levels and mental health status is not optimistic.Psychological,behavioral and physiological indicators have consistent assessment of stress and salivary cortisol can be used as an objective physiological indicator for assessing stress levels.Salivary immunochromatographic test strips with cortisol as the detection target and nano-carbon as the labeler are suitable for rapid detection of stress,which is worthy of popularization.
