Comparison Of Different Doses Of Nalbuphine Combined With Sufentanil For Postoperative Analgesia

Objective:To analysis of postoperative analgesia efFect of nalbuphine combined with sufentanil for laparoscopic hysterectomy.Methods:Selecting 120 patients with laparoscopic hysterectomy under general anesthesia and voluntarily requiring PCIA from August 2017 to September 2018 in Yanbian University Hospital,ASA grade Ⅰ to Ⅱ.They were randomly divided into four groups,the control group was group A,and the experimental group was group B,C,and D,and 30 patients were selected for each group.Proportion of drugs for electronic analgesia pumps:group A was sufentanil 2.0μg/kg+azasetron 10mg,group B was nalbuphine 0.6mg/kg+sufentanil 1.0μg/kg+azasetron 10mg,group C was nalbuphine 1.0mg/kg+sufentanil 1.0μg/kg+ azasetron 10mg,group D was nalbuphine 1.4mg/kg+sufentanil 1.0μg/kg+ azasetron 10mg.The electronic analgesia pump set the parameter background dose to 2.0ml/h,the additional dose to 2.0ml/time,and the locking time to 15min.The VAS scores,RSS scores and BCS scores of the four groups were evaluated at 2h,4h,8h,12h and 24h after operation.At the same time,the patient’s PCIA additional frequency,postoperative nausea,vomiting,agitation,skin itching and other adverse reactions,the patients satisfaction with PCIA.Result:(1)The VAS scores at 2h,4h,8h,12h and 24h after surgery,group A were compared with Group B,C,and D,the VAS score of group B was higher than that of group A at each time point with statistical difference(P<0.05);Compared with group C and D,the VAS scores of group C and group D were lower than those of group B which were statistically different(P<0.05).(2)The RSS score at 2h,4h,8h,12h and 24h after surgery,group A were compared with Group B,C and D,the RSS scores of the C and D groups were higher than those of the group A at each time point which were statistically different(P<0.05);Compared with group C and group D,the scores of RSS in group C and group D were higher than those in group B which were statistically differentCP<0.05);Group C and group D compared RSS scores at each time point with statistical difference(P^<0.05).(3)The BCS score at 2h,4h,8h,12h and 24h after surgery,group A were compared with Group B,C and D,BCS scores in group B were lower than group A at each time point with statistical difference(P<0.05);Compared with group C and group D,the BCS scores of group C and group D were higher than those of group B with statistical difference(P<0.05).(4)The incidence of postoperative nausea in group A was significantly lower than that in group B,C and D(P<0.05);The total incidence of adverse reactions in group B,C and D was lower than that in group A with statistical difference(P<0.05).(5)PCIA additional times,group A was compared with groups B,C and D,and group A was significantly different from group B(P<0.05).(6)PCIA satisfaction,group A was compared with group B,C and D,group C and D were higher than group A with statistical difference(P<0.05);Group C was significantly different from group D(P<0.05).Conclusion:The combination of nalbuphine+sufentanil+azastanide for PCIA after laparoscopic hysterectomy can not only get effective analgesia,but also reduce the incidence of adverse reactions with a high degree of patient satisfaction.
