Effects Of High-flow Nasal Cannula Oxygen Therapy Versus Non-invasive Positive Pressure Ventilation In Patients After Coronary Artery Bypass Grafting

Purpose: To compare the effects of high-flow nasal cannula(HFNC)oxygen therapy versus noninvasive positive pressure ventilation(NPPV)on patients with hyoxemia after coronary artery bypass grafting.Methods: During December 1,2017 and June 1,2018,patients admitted to intensive care unit(ICU)who met the inclusion criteria were randomly divided into HFNC and NPPV group.The cases of treatment failure(reintubation,transferred to other respiratory support or early termination),Brog score,improved Kolcaba comfort score and length of ICU stay and length of hospital stay were recorded.SPSS 22.0 was used to compare and analyze the data of the two groups of patients.Result: A total of 36 patients who met the study criteria were included,including 18 patients in the HFNC group and 18 patients in the NPPV group.The number of treatment failures in HFNC group and NPPV group were 4/18(22.2%)and 3/18(16.7%)respectively,P = 0.674.After treatment,Brog dyspnea score was reduced,and improved Kolcaba comfort status score was significantly higher in the HFNC group than in the NPPV group(P<0.05).The length of ICU stay in the HFNC group and the NPPV group was 6.83±1.97 days and 6.22±1.47 days respectively,P=0.301,and the length of hospital stay was14.67±3.01 days and 15.72±1.96 days,respectively,P=0.221.Conclusions: For adults with hyoxemia after coronary artery bypass grafting,there is no statistical significance in treatment failure rate and improvement of dyspnea between high-flow nasal cannula oxygen therapy and noninvasive positive pressure ventilation.But high-flow nasal cannula oxygen therapy increases patient comfort and thus can be seen as an alternative for adults with hyoxemia after coronary artery bypass grafting.
