| OBJECTIVEWe aim to know the status of medication adherence in Crohn’s Disease(CD)patients with immunosuppressant,and to focus on drug self-management experience in patients with Crohn’s disease during drug maintenance remission treatment,and to further explore the influencing factors affecting drug compliance in patients with Crohn’s disease.METHODSOne hundred and sixteen cases of CD patients were surveyed by social support revalued scale and Medication compliance scale.Describe the current status of compliance with immunosuppressive agents in postoperative CD patients and screen factors that influence the adherence behavior of patients with Crohn’s disease.1.Adoption of a convenient sampling method to select patients with Crohn’s disease diagnosed at the Inflammatory Bowel Diseases Treatment Center of Nanjing General Hospital from December 2015 to October 2016,and to describe the current use of immunosuppressive agents in patients with CD.Status Quo,Screening Factors Affecting Compliance in Crohn’s Disease Patients.2.Using the objective sampling method,using phenomenological and qualitative research methods,through face-to-face semi-structured in-depth interviews,we conducted a study on CD patients meeting the criteria to thoroughly discuss the influencing factors affecting the compliance behavior of immunosuppressive agents in patients with Crohn’s disease.Build a drug management plan to provide the basis.3.This study is a randomized controlled trial in which the experimental group was administered medication via We Chat APP.The control group was routinely followed up by gastrointestinal doctors.Therefore,the subjects were given before,1st and 3rd months after the intervention.The sixth month of data collection.RESULTS1.Crohn’s disease patients after oral azathioprine compliance score of 6.5 ± 1.76,of which 28.44 percent of the patients are in compliance with a lower level,age,gender,disease duration,level of social support affect medication compliance of patients.2.Qualitative interview results revealed that factors that influence medication adherence include: patient-related causes,treatment-related causes,family/social factors,and medical system-related causes.3.There was no significant difference in the scores of MMAS-8 between the two groups for one month or two months.The MMAS-8 scores were statistically different between the fourth and sixth months of intervention.(P<0.05)4.In the We Chat group,the 6-TGN plasma concentration at the 6th month was higher than the 6th-TGN plasma concentration at the 1st month.The mean fecal calprotectin level at the 6th month was lower than the fecal calprotectin concentration at the 1st month.CONCLUSIONThe results of this study show that the compliance of CD patients is at a medium level,and there are many reasons that affect the adherence of CD patients.Among them,age,gender,disease duration,and social support all influence drug compliance.The nurse-led social network APP can improve the adherence of azathioprine in patients with Crohn’s disease.At the same time,the improvement of compliance can reduce the inflammatory index of CD patients,reduce the disease activity,and to a certain extent,reduce the clinical recurrence. |
