OBJECTIVE:To investigate the expression and clinical significance of fibroblast growth factor receptor1(FGFR1)in chronic obstructive pulmonary disease(COPD)and non-small cell lung cancer(NSCLC),and to elucidate the correlation between fibroblast growth factor receptor 1 and COPD and NSCLC.To provide evidence for the prevention,diagnosis and treatment of COPD combined with NSCLC.METHODS:A total of 120 patients with COPD combined with NSCLC and NSCLC who were over 45 years old in Qilu Hospital of Shandong University from January 2014 to January 2016 were divided into COPD combined with NSCLC group,NSCLC group and NSCLC group as normal tissue control group.Among them,49 patients(49 smokers and 0 non-smokers),41 males and 8 females,19 adenocarcinomas,28 squamous cell carcinomas,2 other pathological types of NSCLC,13 patients under 60 years of age,36 patients over 60 years of age,19 patients with lymph node metastasis and 30 patients without lymph node metastasis)were in COPD combined with NSCLC group.There were 71 patients in NSCLC group(45 smokers and 26 non-smokers;30 squamous cell carcinomas,37 adenocarcinomas,4 other pathological types of NSCLC;52 males and 19 females;18 patients under 60 years old,53 patients over 60 years old;26 patients with lymph node metastasis and 45 patients without lymph node metastasis).The expression of FGFR1 in COPD combined with NSCLC,NSCLC and normal tissues adjacent to cancer was studied by immunohistochemistry.Kaplan-Meier survival analysis and COX regression model were used to analyze the independent prognostic factors of COPD patients with NSCLC.RESULTS:In COPD combined with NSCLC group,there were 20 cases with positive expression of fibroblast growth factor receptor1(16 squamous cell carcinomas and 4 adenocarcinomas);13 cases with positive expression of fibroblast growth factor receptor1(9 squamous cell carcinomas and 4 adenocarcinomas)in simple NSCLC group;and 3 cases with positive expression of fibroblast growth factor receptorl in normal control group.1.The positive expression rate of fibroblast growth factor receptorl was 40.82%in COPD combined with NSCLC group,18.31%in NSCLC group and 4.23%in normal control group.The expression of FGFR1 was as follows:COPD combined with NSCLC group>NSCLC group>normal control group.P<0.05.2.The positive expression rate of fibroblast growth factor receptorl in squamous cell carcinoma was 57.14%and in adenocarcinoma was 21.05%respectively in the COPD combined with NSCLC group.The expression of fibroblast growth factor receptorl in squamous cell carcinoma was higher than that in adenocarcinoma.P<0.05.3.In NSCLC group,the positive rate of fibroblast growth factor receptorl was 30%in squamous cell carcinoma and 10.81%in adenocarcinoma.The expression of fibroblast growth factor receptorl in squamous cell carcinoma was higher than that in adenocarcinoma.P<0.05.4.The positive expression rate of fibroblast growth factor receptor1 in squamous cell carcinoma was 57.14%in COPD combined with NSCLC group and 30%in NSCLC group alone.The expression of FGFR1 in COPD combined with NSCLC was higher than that in NSCLC.P<0.05.5.The positive expression rate of fibroblast growth factor receptor1 in adenocarcinoma of COPD combined with NSCLC group was 21.05%,and that in adenocarcinoma of NSCLC group was 10.81%.There was no significant difference in the expression of FGFR1 in two groups of adenocarcinomas.P>0.05.6.The positive expression rate of fibroblast growth factor receptorl in adenocarcinoma of COPD combined with NSCLC group was 21.05%.The positive rate of fibroblast growth factor receptorl in smoking adenocarcinoma and non-smoking adenocarcinoma was 18.18%and 7.69%respectively in NSCLC group.In adenocarcinoma There was no significant difference of the expression of FGFR1 in COPD with NSCLC,smoking with NSCLC and non-smoking with NSCLC.P>0.05.7.The expression of FGFR1 is an independent prognostic factor in COPD combined with NSCLC.The median overall survival time of patients with positive expression of fibroblast growth factor receptorl was shorter than that of patients with negative expression of fibroblast growth factor receptorl.P<0.05.8.There was no significant difference between the expression of FGFR1 and the prognosis of NSCLC.P>0.05.Conclusion:FGFR1 is associated with the onset of COPD and NSCLC,and is an independent adverse prognostic factor for COPD with NSCLC.
