BackgroundThe burden of disease caused by high blood pressure is an important public health challenge in our country.Complications such as stroke,coronary heart disease,and hypertensive nephropathy caused by hypertension lead to high disease death and disability.The study pointed out that scientific and rational treatment of hypertension can effectively reduce its complications and improve the quality of life of patients,and drug treatments of hypertension is an important measure to prevent and control hypertension.At present,the control of hypertension in China is still not optimistic,and the urban-rural difference is significant.The rural control rate is lower than that of the city,and poor medication compliance is the main cause of poor blood pressure control in rural areas.Therefore,it is particularly important to analyze the current status of drug compliance in hypertensive patients in rural areas,to explore the relevant risk factors that affect their compliance,and to improve blood pressure control.Although there are many studies on drug compliance in the hypertension population in China,few studies based on the Andersen model and the path analysis model to explore the influencing factorsObjectivesTo do so,we have the following objectives.First,we assess the quo of medication compliance in rural hypertensive patients.Second,we explore the influencing factors of medication compliance in hypertension population.Lastly,we provide relevant evidence for relevant departments to improve the compliance of drug compliance among high blood pressure people,thereby improving the health status of rural hypertensive patients.MethodsThe study covered 17 cities in Shandong Province,using a three-stage stratified cluster sampling method,a total of 3911 effective samples were taken in two urban areas and 4 rural counties,including 3113 rural hypertension patients.Using Epidata3.1 and SPSS22.0 for data analysis,descriptive analysis to understand the basic situation of medication adherence in rural hypertension population,using univariate logistic regression analysis to identify variables related to medication compliance,multivariate Logistics Regression analysis explains the relevant influencing factors.Finally,use Mplus7.1 to build a path analysis model and explore the relationship between related variables.ResultsThe adherence of medication in rural hypertensive patients in Shandong Province was 44.9%,and the degree of influence on medication compliance in hypertension was promotion factors,propensity factors,health behavior factors and demand factors.Among the promoting factors were family income(P<0.001),health education(P<0.001),and blood pressure measurement frequency(P<0.05).Demand factors included hospitalization(P<0.001)and hypertension comorbidity(P<0.001),hypertension stage(P<0.05)and duration of hypertension(P<0.01).The predisposing factors included gender(P<0.01),education level(P<0.001),K10(P<0.01),and health behavior factors is physical activity(P<0.05).Path analysis showed that gender,K10,and smoking status had a direct effect on medication compliance,gender was the intermediate variable of K10’s effect on medication compliance,and smoking status regulated the role of gender as a mediator of K10 and medication adherence;the total effect value of self-rated health on medication compliance wasβ=0.01(P<0.01),and the direct effect value was β=0.004(P<0.01).Indirect effect value was β=0.004(P<0.01).The total effect of hospitalization on medication compliance was β=0.132(P<0.001),and the direct effect value was β=0.132(P<0.001).The total effect value of compliance was β=0.09(P<0.01),and the indirect effect was β=0.123(P<0.001).ConclusionsThe adherence of medication in rural hypertensive patients in Shandong Province is still low,and there is still a gap compared with other developed regions in China.Therefore,relevant measures are taken to improve the medication compliance of patients with hypertension.Recommendations:1)Standardize the health management of high blood pressure people in rural areas.2)Strengthen health education for high blood pressure people,enhance the understanding of hypertension related knowledge among rural hypertension,and improve health awareness;3)Pay attention to the education of mental health problems of hypertensive people and improve their mental health;4)Strengthen interventions on people’s health behaviors,urge patients with hypertension to adopt an active and healthy lifestyle;5)Pay attention to the construction of rural grassroots medical talents,increase talent input,optimize the structure of medical personnel;6)Standardize medication guidance for hypertension,according to patients develop a personalized medication management plan for your own condition.
