A Study On The Causes Of Hypoglycemia In 29 Patients With Type 2 Diabetes Mellitus Undergoing Oral Sulfonylurea Drugs

Objective: In the medical community,it is well known that if diabetes is not effectively treated,its complications are extremely harmful,can cause various organs and systems of patients with pathological changes,eventually leading to disability or death of patients.However,many people do not understand the dangers of hypoglycemia in diabetics.In fact,severe hypoglycemia can reduce the energy supply of important organs in patients in a short period of time,resulting in dysfunction,such as mental disorders,acute myocardial infarction,acute cerebral infarction,and even lead to death.The purpose of this study is to explore the possible causes and prevention strategies of hypoglycemia in diabetic patients hospitalized for hypoglycemia,in order to guide doctors and patients in clinical work,and to work together to reduce the incidence of hypoglycemia and protect patients’ health.Methods: A retrospective analysis was made on 693 diabetic inpatients in General Hospital of Angang Group from May 1,2016 to September 1,2017.Among them,29 cases were hypoglycemia caused by oral administration of 2 mg glimepiride(amori).29 cases in non-hypoglycemic group were randomly selected for oral administration of 2 mg glimepiride(amori)daily.The patient was treated as a control group.And 58 patients were satisfied with fasting and 2 hours postprandial serum C-peptide level > 1.0 ng / ml.By comparing the two groups of patients in age,glycosylated hemoglobin levels,body mass index,renal function,sleep time and other aspects of differences,summed up the possible causes of hypoglycemia.Among them,12 were male and 17 were female,aged 53-86,glycosylated hemoglobin 6.3-7.8%,BMI 18.2 kg/m2-25.5 kg/m2,glomerular filtration rate 28.48-106.78 ml/min,sleep time 5.5-7 hours;non-hypoglycemic group: 14 men,15 women,age 42-84 years,glycosylation Hemoglobin 7.0%-8.5%,BMI 21.6 kg / m2-25.6 kg / m2,glomerular filtration rate 65.96-114.87 ml / min,sleep time 4.5-8 hours.Through two independent samples t test analysis,Spearman grade correlation analysis,multivariate logistic regression analysis,P < 0.05 as the difference was statistically significant,clear the above factors on the incidence of hypoglycemia in patients with diabetes mellitus.Results: Spearman rank correlation analysis showed that age was positively correlated with hypoglycemia.Hb A1 c level,BMI and glomerular filtration rate were negatively correlated with hypoglycemia.Sleep time was not correlated with hypoglycemia.Multivariate logistic regression analysis showed that only glycosylated hemoglobin and body mass index were negatively correlated with the occurrence of hypoglycemia,while the other indexes,including patient’s age,glomerular filtration rate and sleep time,were not statistically significant.Life is not related.The cause of hypoglycemia is not single,but the result of multiple factors.Conclusion: Through the clinical practice of diagnosis and treatment of hypoglycemia patients,this study summarizes a certain number of cases,through statistical analysis of data indicators,to identify the possible causes of hypoglycemia.Although the results of Spearman rank correlation analysis and multivariate logistic regression analysis are inconsistent,the conclusions can be appropriately adopted considering the small sample size and the confounding factors.Later in the propaganda and education of diabetic patients,we can have a target,avoid the high-risk factors of inducing hypoglycemia,so as to prevent the occurrence of hypoglycemia,not only to protect the lives and health of patients,but also to save treatment costs,and ultimately to improve social and economic benefits of the fundamental purpose.
