Objective: To study the hypoglycemia of diabetic inpatients and predisposing factors,evaluate the level of serum glucose fluctuation index 1,5-AG and oxidative stress indexes 8-iso-PGF2α and SOD,and to explore the correlation between them.Methods: There are 350 diabetic inpatients in this study,292 cases were selected and 58 cases were excluded.According to the medical history and diabetes antibody test,the patients were divided into 3 groups T1 DM,LADA and T2 DM.According to the retrospective survey results,the patients were divided into patients without hypoglycemia and hypoglycemia group(Include patients with severe hypoglycemia,symptomatic hypoglycemia,asymptomatic hypoglycemia,suspicious hypoglycemia).According to Hb A1 c,the patients were divided into the standard-control group(Hb A1 c ≤ 7%),the unstandard-control(7%< Hb A1c≤9%)and the poor-control group(Hb A1c> 9%).The patients were followed up after 3 months.To compare and analyse the general,hypoglycemic or self-monitoring of blood glucose data and laboratory indexes of group T1 DM,LADA and T2 DM.To compare and analyse the general data and laboratory indexes between group hypoglycemia and group non-hypoglycemia.To compare the level of Hb A1 c and the incidence of hypoglycemia after 3 months of follow-up among group T1 DM,LADA and T2 DM.To determine the level of 1,5-AG by Elisa and pyranose oxidase method.To compare the incidence of hypoglycemia and the levels of indicators for blood glucose monitoring among standard-control,unstandard-control and poor-control group.To compare the different levels of 1,5-AG between before and after inhospitalization.To compare the levels of indicators for blood glucose monitoring,8-iso-PGF2α and SOD among group T1 DM,T2DM and LADA and to explore the correlation between them.Results: 1.The incidence of hypoglycemia varies among patients with different types of diabetes,the incidence of hypoglycemia in group T1 DM and LADA were significantly higher than group T2 DM.2.The happening of hypoglycemia was more frequently at night.The common causes of hypoglycemia included irregular eating,excess exercise and so on.3.The incidence of hypoglycemia was higher in patients with lower BMI and more frequently insulin injections.4.The levels of Hb A1 c and the incidence of hypoglycemia were significantly improved after 3 months of follow-up,nocturnal hypoglycemia are worthy of concern.5.The incidence of hypoglycemia increased significantly fllowing with the glycemic control standard.6.When Hb A1c≤7%,the level of 1,5-AG in hypoglycemic group was higher.Higher levels of 1,5-AG could indirectly indicate hypoglycemia and glycemic fluctuations when Hb A1 c was up to standard,which could be used to assess glycemic control together with Hb A1 c.7.There was a significantly correlation between 1,5-AG and 8-iso-PGF2α in group T2 DM and LADA.Conclusion: 1.50.7% inpatients were occurred with hypoglycemia in this study,the incidence of hypoglycemia in group T1 DM and LADA were significantly higher.The happening of hypoglycemia was more frequently at night.The common causes of hypoglycemia included irregular eating,excess exercise and so on.2.The incidence of hypoglycemia increased significantly fllowing with the glycemic control standard.Higher levels of 1,5-AG would indirectly indicate hypoglycemia and glycemic fluctuations when Hb A1 c is up to standard,which could be a clinically significance of monitoring indicators.3.There was a significantly correlation between 1,5-AG and 8-iso-PGF2α in group T2 DM and LADA.Determining the levels of 1,5-AG is contributed to assessing oxidative stress.
