Background: Breast cancer is one of the most common malignant tumors in women, which is a threat to women’s health.Over the years, researchers and clinical workers have explored the etiology of breast cancer and found that the incidence of breast cancer is related to many risk factors, and more than 70% of breast cancer is attributed to environmental factors. Many endocrine pollutants and carcinogens have been found in the environment.Organophosphate esters(OPEs),as a newly developed flame retardant, are widely used in the production of commercial products. However, due to its instability,it is easy to spread to the natural environment. The OPEs in these environments enter the body through food ingestion, skin contact, and inhalation.Researchers have detected OPEs in human breast milk, placenta and serum.Researchers have also done a lot of research on the toxic effects of OPEs,and found that OPEs has potential carcinogenicity, endocrine system interference and neurotoxicity. So it’s allthe more important to learn about the potential hazards that OPEs poses to people. Objective: This study is based on a case-control study,By detecting the serum exposure levels of OPEs in a female breast cancer patient and a matched healthy female patient,in combination with other risk factors related to breast cancer, this study explores the relationship between exposure level and breast cancer risk.It can provide reference for environmental factors and clinical diagnosis and treatment of breast cancer. Methods: By collecting in June 2017-June 2018 for breast nodules pathologic diagnosis of breast cancer during the period of 60 patients, and collect the 60 cases of healthy women serum, collected their information, including general characteristics(age, smoking, drinking and exercise, etc.),menstrual history, history of benign breast disease and family history of malignancy, and gas-chromatography tandem instrument were used to detect the content of serum OPEs.Results: The mean age of the case group and the control group was53.56 ± 7.19 years old and 53.24 ± 6.29 years old, respectively. The BMI of the case group was 23.29 ± 2.39,and that of the control group was 22.78 ± 2.01.There was no significant difference in age and BMI between the two groups(P > 0.05).The age of menarche in the case group and control group was 16.23 ± 1.43 years old and 15.52 ± 1.68 yearsold, respectively. The difference of menarche age in the two groups was statistically significant(t=2.51,p < 0.05).BMI grouping, alcohol consumption, smoking and history of benign diseases of the breast, physical exercise, age of menarche, history of lactation, regularity of menstruation and menopause were not significantly different between the two groups in this study, and the difference of family malignant tumor history between the case group and the control group was statistically significant(p < 0.05).The6 OPEs detection rates ranged from 26.7% to 78.3% in case group and18.3-76.7% in control group,respectively.Tris(2-choroethyl)phosphate,(TCEP)and Triphenyl phosphate(TPhP)levels higher than the control group, with statistical difference(p < 0.05).Tri-n-propyl phosphate,(TPrP),Tri-n-butyl phosphate,(TnBP),2-Ethylhexyl diphenyl phosphate(EHDP)and Tri-iso-butyl phosphate(TIBP)levels not seen in the object of study differences between groups. To estimate the relationship between exposure levels and breast cancer risk.We divided all subjects into high and low eight exposure levels according to the upper quartile of serum OPEs(P75),and introduced Logistic regression analysis. After selection, age of menarche(p=0.14),family history of malignancy(p=0.05)and menstrual pattern(p=0.11)were included as variables in the equation. In the case group and the control group of OPEs in five kinds of OPEs the OR values were greater than 1,press the OR value from big to small order respectively TPh P > TCEP >TPr P > TIBP > Tn BP,However, only TPh P and TCEP showed significant positive correlation(p < 0.05),indicating that TPh P and TCEP high exposure levels, the risk of breast cancer will increase(OR=3.04 for TPh P,OR=2.28 for TCEP).Conclusion: 1.OPEs exposure level may be related to the occurrence of breast cancer, among which TPh P and TCEP exposure levels are significantly positively correlated with the occurrence of breast cancer.2.Combined with the results of this study and other experimental literature,we can draw the conclusion that the serum sample is suitable for the detection of OPEs exposure level in the population due to its easy access and large population size.Reducing the exposure of OPEs may play a role in preventing or delaying the development of breast cancer.
